Pro: anesthesia for children in the developing world should be delivered by medical anesthetists.
Outcomes from pediatric surgery when carried out by trained pediatric anesthetists are excellent. This is not the case when the anesthesia provider is poorly trained. ⋯ These problems should be addressed but safety of the children should not be compromised. One should not condone poor standards to deprive children of safe anesthesia and the caregiver of much needed self-respect.

Pro: pediatric anesthesia training in developing countries is best achieved by selective out of country scholarships.
Pediatric anesthesia training in developing countries is best achieved by out of country scholarships rather than structured outreach visits by teams of specialists from the developed world. Although this may seem an expensive option with slow return, it is the only sustainable way to train future generations of specialized pediatric anesthetists in developing countries.
