Over-screening for chlamydia and gonorrhea among urban women age ≥25 years.
The purpose of this study was to determine the prevalence of Chlamydia trachomatis (CT) and Neisseria gonorrhoeae (NG) in older women who are at increased risk of sexually transmitted infections (STIs) based on community STI prevalence. Additionally, we explored the associations between STI risk factors and CT/NG to determine the percentage of women who were over-screened. ⋯ CT/NG prevalence among older women was low, even in a community of high STI prevalence. More than 20% of women could have avoided CT/NG evaluation without impacting the detection of positive results in our clinic cohort. Over-screening occurred among asymptomatic, non-STI-exposed women who were menopausal, had a hysterectomy, and were >40 years old.

Randomized Controlled TrialPostpartum pelvic floor muscle training and pelvic organ prolapse--a randomized trial of primiparous women.
Pelvic organ prolapse (POP) is a common and distressing condition. The aim of the present study was to evaluate the effect of pelvic floor muscle training (PFMT) on prevention and treatment of symptoms and signs of POP in primiparous postpartum women. ⋯ No effect was found of postpartum PFMT on POP in primiparous women. More randomized controlled trials are needed before strong conclusions can be drawn on the effect of PFMT on POP in the particular population.

Medical and obstetric complications among pregnant women with cystic fibrosis.
The objective of this study was to estimate the nationwide prevalence of cystic fibrosis (CF) in pregnancy and determine what medical complications exist at delivery among pregnant women with CF. ⋯ Pregnant women with CF are more likely to die, require mechanical ventilation, and have infectious complications compared with women without CF, although the absolute risks are low and these events are relatively rare.

Uptake of noninvasive prenatal testing at a large academic referral center.
Noninvasive prenatal testing (NIPT) is a recently developed risk-assessment technique with high sensitivity and specificity for fetal aneuploidy. The effect NIPT has had on traditional screening and diagnostic testing has not been clearly demonstrated. In this study, NIPT uptake and subsequent changes in the utilization of first-trimester screen (FTS), chorionic villus sampling (CVS), and amniocentesis in a single referral center is reported. ⋯ NIPT was quickly adopted by our high-risk patient population, and significantly decreased alternate prenatal screening and diagnostic testing in a short period of time.

Performance of racial and ethnic minority-serving hospitals on delivery-related indicators.
We sought to explore how racial/ethnic minority-serving hospitals perform on 15 delivery-related indicators, and examine whether indicators vary by race/ethnicity within the same type of hospitals. ⋯ We found considerable differences in delivery-related indicators by hospital type and patients' race/ethnicity. Obstetric care quality measures are needed to track racial/ethnic disparities at the facility and population levels.
