Randomized Controlled Trial Clinical TrialThe role of irrigation in the development of hypothermia during laparoscopic surgery.
Our purpose was to determine the incidence and etiology of hypothermia during laparoscopic surgery and to evaluate the role of irrigation fluid temperature. ⋯ Hypothermia is extremely common in laparoscopic surgery and is related to the length of anesthesia and the use of ambient temperature irrigation fluid. The use of warmed irrigation fluid can decrease, but not eliminate, this drop in core temperature.

The McRoberts' maneuver for the alleviation of shoulder dystocia: how successful is it?
Our purpose was to determine the rate of success of the McRoberts' maneuver as the initial treatment for shoulder dystocia and to compare the rate of maternal and neonatal morbidity with those cases of shoulder dystocia requiring additional obstetric maneuvers. A secondary goal was to assess those factors associated with successful McRoberts' maneuvers. ⋯ These data suggest that the McRoberts' maneuver is associated with a significant degree of success in relieving shoulder dystocia and may be associated with decreased morbidity compared with other maneuvers. On the basis of these findings, we recommend the McRoberts' maneuver as the initial technique for disimpaction of the anterior shoulder.

New colposcopic protocols allow examiners to better document genital trauma in rape victims. We report our findings on the locations and types of genital injury seen in female assault victims versus women engaging in consensual sex. ⋯ A localized pattern of genital trauma can frequently be seen in women reporting nonconsensual sexual intercourse; such findings are useful for the clinical forensic examiner.

Our purpose was to determine the effect of smoking during pregnancy on neonatal hemostasis. ⋯ The balance between the components of coagulation and fibrinolytic pathways in neonates is not disturbed by maternal smoking.

Blood pressure measurement in pregnant women in the left lateral recumbent position.
To evaluate blood pressure in pregnant women in the left lateral position, we studied indirect blood pressure in 169 patients with normal blood pressure, chronic hypertension, and preeclampsia in the supine and then the lateral recumbent positions. Two additional patients had aortic arch blood pressure compared with indirect measures. ⋯ Regarding direct aortic arch blood pressure, (1) supine blood pressure equaled that in the lateral position and (2) direct blood pressure in the lateral position equaled the mean indirect mean arterial pressure of both arms. Therefore the actual blood pressure in the lateral recumbent position is the combined mean arterial pressure of both arms.
