CT changes after trochleoplasty for symptomatic trochlear dysplasia.
Trochlear dysplasia is an important risk factor for patellar instability. Because of a decreased trochlear depth in combination with a low lateral femoral condyle, the patella cannot engage properly in the trochlea. Trochleoplasty is a surgical procedure, which strives to correct such bony abnormalities. ⋯ In the CT scan patients with trochlear dysplasia demonstrated a poor depth, or even a flat or convex trochlea with a greater sulcus and lateral trochlear slope angle, a lateralised patella to the trochlear groove with poor congruency, and a greater lateral patellar inclination angle. Trochleoplasty can correct the pathological features of trochlear dysplasia by surgically creating more normal anatomy. The goal of this surgical procedure is to steepen and lateralise the trochlear groove for a better engagement of the patella.

Review Case ReportsSimultaneous bilateral tibial tubercle avulsion fracture in an adolescent: a case report and review of the literature.
Avulsion fractures of the tibial tubercle are uncommon. Bilateral tibial tubercle avulsion fractures are extremely rare. ⋯ We report here on the successful surgical treatment of a simultaneous bilateral fracture of the tibial tuberosity in an adolescent. These types of fractures involve a growth plate, extend through the articular surface and appear to do well with open reduction and secure internal fixation despite their bilateral nature, with recovery and functional outcome comparable to results from unilateral tibial tubercle avulsion fractures.

Arthroscopic treatment of patients with moderate arthrofibrosis after total knee replacement.
The purpose of this study was to document the effect of arthroscopic management in patients with knee stiffness after total knee replacement. We present a case series study, in which 32 patients have been treated for moderate arthrofibrosis of the knee after total knee replacement, with the same regimen. We have excluded all cases of stiffness, because of infection, mechanical mal-alignment, loosening of the implants and other obvious reasons of stiffness of the knee, rather than pure arthrofibrosis. ⋯ The average pain scores improved from 30 points pre-operatively to 41 at the time of final follow up. Our results showed that arthroscopic management of knee stiffness following total knee replacement is a safe and efficient method of treatment. Pain and functional knee scores can improve markedly.

Synovial shelves of the knee: association with chondral lesions.
The objectives of the present study were to evaluate whether synovial shelves of the knee are associated with increased incidence of chondral lesions and to determine which types of plica are associated with significant articular damage. Data were collected prospectively from 1,000 consecutive knee arthroscopies. Of these patients, 321 (32.1%) were found to have synovial shelves of the knee. ⋯ In conclusion, synovial shelves of the knee are associated with an increased incidence of cartilage lesions. Larger shelves, particularly with chronic inflammation are associated with more frequently occurring and larger articular lesions. Areas with increased incidence of chondral lesions, in patients with plicae, include particularly the lower patella and the non-weight-bearing medial femoral condyle.

Tibial press-fit fixation of the hamstring tendons for ACL-reconstruction.
Press-fit fixation of patellar tendon bone anterior cruciate ligament autografts is an interesting technique because no hardware is necessary to achieve fixation. Up till the present point, there is no biomechanical data available for the tibial press-fit fixation of the hamstring tendons. Hamstring tendons of 21 human cadavers (age: 41.9 +/- 13.1 years) were used. ⋯ Only one technique (T) showed satisfactory elongation behavior during long-term cyclic loading. Interference screw fixation demonstrated significantly higher stiffness. Only one of the investigated techniques (T) seemed to exhibit adequate mechanical properties necessary for early aggressive rehabilitation programs.
