Prevalence of Dangerous Ethmoid in a Tertiary Center in Eastern Nepal.
Endoscopic sinus surgery is a well-known approach for sino-nasal pathologies. Due to close proximity to the brain and orbit, the surgeon should be aware of the sino-nasal anatomy and its associated variations. Detailed preoperative assessment of the sinus computed tomography scans reduces the frequency of severe complications in patients undergoing an endoscopic sinus surgery. So, the aim of this study is to find the prevalence of dangerous ethmoid in a tertiary center in eastern Nepal. ⋯ The most common type of dangerous ethmoid was keros type II.

Case ReportsAcute Stroke in Tubercular Meningitis: A Case Report.
Most of the strokes in tubercular meningitis are multiple, bilateral, and located in the basal ganglia, especially the 'tubercular zone', which comprises of the caudate, anterior thalamus, anterior limb, and genu of the internal capsule. These are attributed to the involvement of medial striate, thalamotuberal, and thalamostriate arteries, which are embedded in exudates and likely to be stretched by coexistent hydrocephalus. Corticosteroids with antitubercular therapy were thought to reduce mortality and morbidity but their role in lowering strokes has not been proven. ⋯ Here, we are reporting a case of 22-years female patient with tubercular meningitis. She had complications of ischemic infarct and severe communicating hydrocephalus with a seizure disorder. Keywords: meningeal tuberculosis; stroke; tuberculoma; vasculitis.

Prevalence of Dyspnea among Patients Attending the Emergency Department of a Tertiary Care Hospital: A Descriptive Cross-sectional Study.
Dyspnea is a common presenting complaint in the emergency department worldwide and a diagnostic challenge for emergency physicians. Our study aims to find the prevalence of dyspnea among patients attending emergency department in our hospital. ⋯ The prevalence of dyspnea among patients attending emergency department of our hospital is higher compared to that of other studies. This warrants structured and prompt management of dyspnea for quality improvement.

Case ReportsAlpha-gal Allergy in a 6-Year-Old Male: A Case Report.
Alpha-gal allergy is a reaction where the immunoglobulin E antibody elicits a response to galactosealpha-1,3-galactose (alpha-gal) which is a mammalian oligosaccharide epitope found in nonprimate mammalian. After being exposed to a tick bite, particularly the Lone Star tick (Amblyomma americanum), an individual has been known to develop an alpha-gal allergy. Our patient presented with symptoms of delayed-onset allergy 3-8 hours after consuming mammalian meat products including beef and pork. ⋯ Our patient highlights one of the few reported childhood cases with an alpha-gal allergy. We recommend a careful history, in particular, asking if the patient has a past history of tick bites and the appearance of allergy symptoms following beef or pork consumption. Prevalence of Substance Use among Undergraduate Students in a Medical College of Nepal.
The consumption of substances is a common practice among the medical students and their use might reduce educational and clinical performances as well as judgment of the students. This study aims to find out the prevalence of substance use among medical students in BP Koirala Institute of Health Sciences. ⋯ The overall substance use among medical students was high compared to other studies. Alcohol was the most common substance misused by the student followed by tobacco and marijuana. Proper counseling and awareness programs about the potential risk of substances are recommended for the betterment of the students.
