Case ReportsChronic Lithium Toxicity in Old Age Patients Taking Angiotensin Receptor Blocker: A Case Report.
Chronic lithium toxicity is a potentially serious side effect on patients taking lithium for a prolonged period with the diagnosis of mood disorders. The toxicity is even higher in patients taking drugs that interfere with the metabolism of lithium like angiotensin receptor blockers and in older patients with reduced kidney function. In this report, we present the case of a 62-year-old woman who presented to the emergency department with symptoms including loose stools, generalised body weakness, slurred speech, coarse hand tremors, and dystonia persisting for fifteen days. She had been under lithium therapy for bipolar type 1 disorder for 15 years before experiencing these symptoms, which emerged shortly after the addition of telmisartan (angiotensinogen receptor blocker) for hypertension. ⋯ angiotensin receptor blocker; bipolar disorder; case reports; lithium; telmisartan.

Hypovitaminosis D among Blood Samples of Patients Presenting to the Department of Biochemistry of a Tertiary Care Center.
Hypovitaminosis D is a global public health problem affecting approximately one billion people, with a particularly high prevalence in South Asia. Several hospital-based studies from Nepal show a high prevalence of hypovitaminosis D. However, large-scale community-based studies are lacking. The aim of the study was to find out the prevalence of hypovitaminosis D among blood samples of patients presenting to the Department of Biochemistry of a tertiary care centre. ⋯ prevalence; vitamin D; vitamin D deficiency.

Special Sensory Function Deficit among Patients with Post-COVID-19 Visiting a Tertiary Care Centre.
Several patients who recover from COVID-19 infection continue to have persistent symptoms even after recovery from the disease. The special sensory functions such as taste, smell and hearing are affected by COVID-19 infection even after recovery from the illness. The aim of the study was to find out the prevalence of special sensory deficits among patients with post-COVID-19 visiting a tertiary care centre. ⋯ anosmia; COVID-19; hearing loss.

Multidrug-resistant among Non-Fermenting Gram-negative Bacteria Isolated in the Department of Microbiology of a Tertiary Care Centre.
Infection caused by Non-fermenting Gram-negative bacteria (NFGNB) like Pseudomonas aeruginosa and Acinetobacter baumannii leads to life-threatening conditions. These bacteria are often multidrug-resistant which leads to limited therapeutic options leading to treatment failure. Little information is available regarding the prevalence and resistance pattern of such bacteria in our country. The aim of the study was to find out the prevalence of multidrug-resistant among non-fermenting Gram-negative bacteria isolated in the Department of Microbiology of a tertiary care centre. ⋯ Acinetobacter baumannii; antibiotic; Pseudomonas aeruginosa; prevalence.

Practising medicine is all about art, skills and knowledge. According to the Tribhuvan University curriculum, the second phase, third year Bachelor of medicine and bachelor of surgery is the beginning of clinical skill learning. After studying integrated basic medical sciences for two years which is more like a conventional method of learning, the third year brings a new experience. It integrates theoretical knowledge in hospital settings by observing the patients, historytaking, examination, learning effective communication skills and empathy. ⋯ clinical skills; communication; empathy.
