Bridging gaps in care: Implementation of a pharmacist-led transitions-of-care program.
The development and implementation of a hospitalwide, pharmacist-led transitions-of-care (TOC) program are described. ⋯ After a quality improvement initiative developed by pharmacists was implemented, HCAHPS scores showed improvement and more patients received medication reconciliation.

Results of an initiative at an academic medical center to reduce prescription opioid use in patients with acute traumatic injuries are reported. ⋯ Targeted provider and patient education on minimizing opioid use was associated with a reduction in MME on discharge from the hospital after traumatic injury.

Evaluation of opioid prescribing after rescheduling of hydrocodone-containing products.
Institutional prescribing trends of hydrocodone-containing products (HCPs) before and after the Drug Enforcement Administration's rescheduling of HCPs were evaluated. ⋯ The rescheduling of HCPs resulted in a reduction in HCP prescriptions but was accompanied by increases in the use of codeine-containing products and tramadol in all settings.
