We constructed a prototype community first responder (CFR) dispatch system. The system sends incident information, including a map, to the chosen CFR's mobile phone. We tested it in a simulation of 30 out-of-hospital cardiac arrest incidents which had occurred in the town of Motegi during the previous year. ⋯ The mean response time (from the CFR receiving dispatch information to arrival at the scene) was 3 min 37s faster than the actual response time in the corresponding historical control, i.e. the response time was reduced by 36% (P < 0.01). The median travel distance of the CFRs was 3.4 km and there was a positive correlation between response time and travel distance. The study showed that interactive communication between dispatcher and CFR was important for effective operation and that CFRs could reach an OHCA patient before the Emergency Medical Service arrives.

We evaluated a real-time, prehospital ultrasound image transmission system for use in focused assessment with sonography for trauma (FAST). The wireless, real-time ultrasound image transmission system comprised an ultrasound scanner with a convex abdominal transducer and a notebook computer connected to a 3 G wireless network for video data transmission. In our simulation experiment, ultrasonography was performed by emergency medical technicians (EMTs) on a human body phantom with simulated haemoperitoneum. ⋯ A total of 21 ultrasound video clips was used and 13 emergency physicians participated in the study. The sensitivity and specificity were 90.0% (95% Confidence Interval, CI, 83.5-94.6) and 85.3% (95% CI 78.4-90.7) respectively, and the accuracy of detecting abnormal ultrasound results was 87.7% (95% CI 83.8-91.6). Diagnosis of hemoperitonuem in trauma patients by an emergency physician based on the transmitted video images of FAST performed by an EMT is feasible, and has an accuracy of about 88%.

We developed, implemented and evaluated a web-based continuing professional development (CPD) programme on medical genetics. Development of the CPD programme followed the ADDIE model, i.e. Analysis, Design, Develop, Implement and Evaluation. ⋯ The programme was piloted on a group of 10 doctors. The average pre-knowledge score was 40.3 and the post-knowledge score was 62.1 marks out of 100 (P = 0.002). We conclude that a web-based CPD programme on medical genetics is feasible in Sri Lanka.

An Emergency Telehealth Service (ETS) was established in August 2012 in Western Australia. High definition videoconferencing was used to deliver emergency telemedicine to rural emergency departments, mainly staffed by nurses. In the first 11 months, over 3000 episodes of care were provided. ⋯ In addition, ETS physicians assisted with clinical coordination and transfer of patients. Almost all teleconsultations (98%) were with nursing staff at sites without medical cover. Expansion of the ETS throughout Western Australia is currently being planned.

We estimated the longer-term cost-effectiveness of using telemedicine screening for prenatal detection of congenital heart disease (CHD). One hospital in south-east England with a telemedicine service was connected to a fetal cardiology unit in London. A UK health service perspective was adopted. ⋯ The model showed that offering telemedicine screening by specialists to all standard-risk pregnant women was the dominant strategy (i.e. cheaper and more effective). The sensitivity analyses found that the model was robust, and that telemedicine remained the most cost-effective strategy. The study showed that it would be cost-effective to provide telemedicine examinations as part of an antenatal screening programme for all standard-risk women.
