The incidence of pneumothorax (PTX) as a complication of computerized tomography guided fine needle aspirates (CT-FNA) of solitary pulmonary nodules (SPN) varies from 8-61%. It has been suggested that the practice of obtaining a delayed chest radiograph in patients who have undergone CT-FNA of SPN is not cost effective and adds little information concerning lung expansion obtained by CT at the end of the procedure. It, however, is not known what percent of patients with a PTX present immediately after CT-FNA but do not require prompt chest tube placement will progress and require intervention later. ⋯ These data suggest that patients who develop a PTX immediately after CT-FNA but who do not require prompt pleural space evacuation should be followed closely both clinically and radiographically.

Comparative Study
QT interval in normal infants during sleep with concurrent evaluation of QT correction formulae.
Physiological changes in the QT interval caused by sleep are important in the evaluation of the normal QT interval. Traditionally, Bazett's formula is used for correcting the QT interval for heart rate in childhood. We set out to establish the QT-RR relationship in infants during sleep and concurrently test the performance of four QT correction formulae to determine their comparability and clinical utility. ⋯ These formulae are strongly dependent on heart rate. In the setting of a borderline QTc Bazett prolongation, Hodges formula may be a useful adjunct. Normal infants may have a slightly longer QTc Bazett during sleep.

Side clamp used during off-pump coronary artery bypass does not increase the risk of stroke.
The purpose of this study was to investigate whether the incidence of stroke after off-pump AC bypass was lower than that after on-pump AC bypass. ⋯ The incidence of postoperative stroke was lower in the off-pump group than in the on-pump group, even though the side clamp was applied to the ascending aorta for aortocoronary bypass. The use of side clamping for off-pump AC bypass does not increase the risk of postoperative stroke.

The purpose of this study was to describe the characteristics and outcome in relation to sex in consecutive patients who were transported by an ambulance due to symptoms arousing suspicion of acute coronary syndrome. ⋯ Our study suggests that among ambulance transported patients with suspicion of acute coronary syndrome there are some differences between women and men in terms of their characteristics and underlying etiology, but not in terms of the risk of mortality.

To evaluate a protocol for rapid sequence intubation (RSI) for pediatric patients in a Level 1 trauma center. ⋯ This protocol based pediatric rapid sequence intubation method worked well in an EM residency program. More intubations were in males and more were necessary due to trauma in this group.
