The purpose of this study was to quantify the effects of rigid syndesmotic fixation on functional talar position and cartilage contact mechanics. ⋯ Two-way ANOVA indicated syndesmotic screw fixation significantly increased contact pressure in the medial/lateral gutters and decreased talar dome contact pressure while minimally altering talar position.

The optimal surgical approach for displaced intra-articular calcaneal fractures (DIACF) is subject of debate. The primary aim of this systematic review and meta-analysis was to assess wound-healing complications following the sinus tarsi approach (STA) compared to the extended lateral approach (ELA). Secondary aims were to assess time to surgery, operative time, calcaneal anatomy restoration, functional outcome, implant removal and injury to the peroneal tendons and sural nerve. ⋯ The STA is associated with significantly less wound healing complications. With similar functional outcome and calcaneal anatomy restoration, the STA may be the preferred approach in the operative treatment of Sanders type II/III DIACF.

Chronic injuries of the distal tibio-fibular syndesmosis often present with non-specific clinical and radiographic findings. If chronic instability to the distal tibio-fibular syndesmosis is verified, various reconstruction options are available. The purpose of this article is to give a systematic review of current surgical treatment options in patients with chronic syndesmotic injury. ⋯ Level IV; Systematic Review of Level IV Studies.

Syndesmotic injures are common and weight bearing imaging studies are often advocated to assess disruption. Although studies have examined the anatomical relationship between the fibula and incisura, the effect of weight-bearing on the syndesmosis has not been well reported. We characterise the changes which occur at the syndesmosis during weight-bearing. ⋯ This is the first study to measure the differences in position of the syndesmosis during weight-bearing in a population of patients that have undergone both weight bearing and non weight bearing CT. Our study confirms that weight-bearing results in lateral and posterior translation, and external rotation of the fibula in relation to the incisura and our findings should help in future studies looking at the effect of weight bearing on syndesmotic pathology.

The purpose is to compare the effectiveness and imaging changes (US and MRI) between PRP and corticoids injections for the treatment of chronic plantar fasciitis, using clinical results evaluated by the visual analogue scale (VAS), the AOFAS clinical rating system and the modified Roles and Maudsley score, and using imaging results (US and MRI). Our hypothesis is that PRP infiltrations are a more effective therapeutic method than infiltrations with corticosteroids. A single-centre, non randomized, prospective study of 40 consecutive patients (40 feet) with plantar fasciitis who had not responded to conservative treatment for at least 6 months was undertaken. ⋯ In group B the change was from 8.05mm to 6.13mm over 3 months, increasing to 6.9mm after 6 months. The other inflammatory signs improved in all cases, especially in group A. The treatment of chronic plantar fasciitis by two injections of PRP is a safe, more efficient and long-lasting method than corticoid injections.
