Self-insured employers cover more people than Medicare, Medicaid, or direct purchasers of private insurance.This study examined the ability of self-insured employers to negotiate hospital prices and the relationship between hospital prices and employer market power in the United States. ⋯ Employer market power is low in most MSAs. Self-insured employers may consider building purchase alliances with state and local government employee groups to enhance their market power and lower negotiated prices for hospital services.

To identify areas where transition from hospital to community could be improved, with a special focus on racial, ethnic, and language differences. ⋯ Our findings highlight the enhanced difficulties that diverse patients may experience when transitioning from hospital to community-based settings. When considering how to best address the complex needs of diverse populations, interventions must be sensitive to the presence or absence of others, potential digital divides, and medical interpretation.

Inflammatory bowel disease (IBD) is commonly associated with iron deficiency anemia (IDA), with a multifactorial pathophysiology. Diagnosis of both IDA and IBD places patients at risk for poor quality of life, as well as increased hospitalization and healthcare utilization. Treatment of IDA involves iron repletion with either oral or intravenous iron products. When selecting therapy, the total expenses of care must be considered, including direct and indirect costs, as well as patient clinical outcomes.
