Epidemiology and aetiology of paediatric traumatic cardiac arrest in England and Wales.
To describe the epidemiology and aetiology of paediatric traumatic cardiac arrest (TCA) in England and Wales. ⋯ Survival is possible from the resuscitation of children in TCA, with overall survival comparable to that reported in adults. The highest survival was observed in those with a pre-hospital only TCA, and those who were transported to an MTC. Early identification and aggressive management of paediatric TCA is advocated.

Restricted fluid bolus volume in early septic shock: results of the Fluids in Shock pilot trial.
To determine the feasibility of Fluids in Shock, a randomised controlled trial (RCT) of restricted fluid bolus volume (10 mL/kg) versus recommended practice (20 mL/kg). ⋯ Participants were not as unwell as expected. A larger trial is not feasible in its current design in the UK.

To understand the case mix of three different paediatric services, reasons for using an acute paediatric service in a region of developing integrated care and where acute attendances could alternatively have been managed. ⋯ One in two attendances to acute paediatric services could have been managed in the community. Integration of paediatric services could help address parental reasons for attending acute services, as well as facilitating the community management of chronic conditions.

To describe the demographics, mechanisms, presentation, injury patterns and outcomes for children with traumatic injuries. ⋯ A common age of injury and mortality was infants aged <1 year. Accident prevention strategies need to focus on the prevention of non-accidental injuries in infants. Trauma services need to be organised to accommodate peak presentation times, which are after school, weekends and the summer.
