Randomized Controlled Trial Multicenter StudyMicrodiscectomy compared with transforaminal epidural steroid injection for persistent radicular pain caused by prolapsed intervertebral disc: the NERVES RCT.
Sciatica is a common condition reported to affect > 3% of the UK population at any time and is most often caused by a prolapsed intervertebral disc. Currently, there is no uniformly adopted treatment strategy. Invasive treatments, such as surgery (i.e. microdiscectomy) and transforaminal epidural steroid injection, are often reserved for failed conservative treatment. ⋯ These results will lead to further studies in the streamlining and earlier management of discogenic sciatica.

Randomized Controlled TrialTranexamic acid to reduce head injury death in people with traumatic brain injury: the CRASH-3 international RCT.
Tranexamic acid safely reduces mortality in traumatic extracranial bleeding. Intracranial bleeding is common after traumatic brain injury and can cause brain herniation and death. We assessed the effects of tranexamic acid in traumatic brain injury patients. ⋯ Time to treatment may have been underestimated.

Randomized Controlled TrialAssistive technology and telecare to maintain independent living at home for people with dementia: the ATTILA RCT.
Assistive technology and telecare have been promoted to manage the risks associated with independent living for people with dementia, but there is limited evidence of their effectiveness. ⋯ Future work could examine whether or not improved assessment that is more personalised to an individual is beneficial.

Modelling of hypothetical SARS-CoV-2 point-of-care tests on admission to hospital from A&E: rapid cost-effectiveness analysis.
Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is the virus that causes coronavirus disease 2019. At the time of writing (October 2020), the number of cases of COVID-19 had been approaching 38 million and more than 1 million deaths were attributable to it. SARS-CoV-2 appears to be highly transmissible and could rapidly spread in hospital wards. ⋯ This report was commissioned by the National Institute for Health Research (NIHR) Evidence Synthesis programme as project number 132154. Meta AnalysisCognitive-behavioural therapy for a variety of conditions: an overview of systematic reviews and panoramic meta-analysis.
Cognitive-behavioural therapy aims to increase quality of life by changing cognitive and behavioural factors that maintain problematic symptoms. A previous overview of cognitive-behavioural therapy systematic reviews suggested that cognitive-behavioural therapy was effective for many conditions.
