In India, health services are funded largely through out-of-pocket spendings (OOPS). We carried out this study to collect data on the cost of an illness episode and parameters affecting cost in five locations in India. ⋯ Interventions to reduce the cost of illness should be context-specific, as there is no "one-size-fits-all" model to establish the cost of healthcare for the entire sub-continent. Aggregated expenses, rather than only hospitalizations, can cause catastrophic consequences of illness.

With the increasing prevalence of type 2 diabetes and cardiovascular disease in Taiwan, the understanding of metabolic syndrome (MetS) becomes more important. The purpose of this study was to investigate the clustering patterns of the risk variables of the MetS with factor analysis (FacAn). ⋯ Our results were consistent with different ethnic groups in earlier reports that more than two dimensions were identified and that the MetS is not unified by a single underlying aetiology, i.e., insulin resistance. Longitudinal analysis in this and other populations will be required to validate our findings and to test their generalisability.

Clinical laboratories need to develop quick screening methods for detection of extended spectrum beta-lactamase (ESBL) producing strains, so that the appropriate medication can be started without delay. In this study, we report the screening sensitivity of four representative antimicrobial agents i.e., cefpodoxime, cefotaxime, ceftazidime and aztreonam, commonly used for ESBL detection in Klebsiella spp. ⋯ Using the standard disk diffusion as screening test for identifying ESBL producers, cefpodoxime was found to be the most efficient antimicrobial agent in screening isolates as potential ESBL producers followed by ceftazidime in Klebsiella spp. isolates.

Production of extended spectrum beta-lactamases (ESBLs) and AmpC beta-lactamases are the most common mechanisms of antimicrobial resistance in Gram negative bacilli. A prospective study was undertaken to know the occurrence of ESBL and AmpC producing strains and their antibiotic susceptibilities to newer agents to guide empirical therapy for complicated urinary tract infections. ⋯ Overall, 22.1 per cent of our isolates were highly drug resistant, and ESBL producers could explain only 36.5 per cent of HDRU in our study. Therefore, we assume that AmpC beta-lactamases are more important in our setting. Based on our finding a test using discs containing piperacillin and piperacillin-tazobactam ( PtPc) disc at a distance of 20 mm would act as a useful screening procedure for AmpC production as AmpC beta-lactamase producers are more susceptible to tazobactam as compared to clavulanic acid.
