Ultrasound Guidance Versus Landmark-Guided Palpation for Radial Arterial Line Placement by Novice Emergency Medicine Interns: A Randomized Controlled Trial.
More than 10 million arterial lines are placed annually worldwide, many of which happen in the emergency department. Before the introduction of point-of-care ultrasound, landmark-guided palpation (LMGP) was considered standard of care. ⋯ USG improved first-pass and overall success of radial arterial line cannulation while reducing time to access and attempts when used by novice emergency medicine interns.

Empyema necessitans is a rare complication of pleural empyema characterized by the dissection of pus through the soft tissues of the chest wall and eventually through the skin. The skin manifestation may appear as a superficial abscess. ⋯ A 63-year-old woman presented to the Emergency Department (ED) with a chief complaint of dyspnea, dry cough, and a cutaneous nodule on her right chest wall. Three weeks prior to her ED visit, she underwent an exploratory thoracotomy and chest tube placement. The chest tube was removed 2 weeks later. Her physical examination was significant for decreased breath sounds over her right lung fields and a painful, fluctuant, and erythematous nodule on the right chest wall where the chest tube had previously been inserted. Externally, the dermal findings appeared to be a superficial abscess. A chest X-ray study showed a large pleural effusion in her right hemithorax. Point-of-care ultrasound (POCUS) performed by an emergency physician showed evidence of a tract extending from the nodule toward the pleural space that led to the correct diagnosis and treatment of empyema necessitans. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: It is important to distinguish between a superficial abscess, which requires local drainage, and empyema necessitans, which requires either chest tube drainage, open drainage, or even decortication in specific cases. In such cases, POCUS can facilitate a rapid, accurate diagnosis, and lead to the correct treatment.

Characteristics of Anaphylactic Reactions: A Prospective Observational Study in Japan.
Anaphylaxis is a systemic, life-threating, allergic reaction in which the clinical features may vary in different populations or due to the allergic triggers. Moreover, the timing and characteristics of biphasic anaphylactic reactions remain unclear. ⋯ In this prospective ED study, foods were the most common triggers of anaphylaxis, and epinephrine administration was associated with a decreased occurrence of biphasic anaphylactic reactions. Over half of patients experiencing biphasic anaphylactic reactions developed symptoms more than 10 h after presentation.

Helicopter Emergency Medical Services (HEMS) dispatch currently depends on predefined protocols, on first responders' initial assessment, or on medical direction decision in some states. National guidelines do not provide recommendations concerning prehospital time criteria. ⋯ In adult patients with blunt trauma, HEMS transport was associated with overall improved survival rates mainly in the first 30 min after injury. GEMS transport, however, had a survival advantage in the 61- to 90-min total prehospital time interval.

Kikuchi disease, also known as Kikuchi-Fujimoto disease, is a rare, benign cause of cervical lymphadenitis. It is characterized by painful cervical lymphadenopathy with fevers, leukopenia, and an elevated erythrocyte sedimentation rate. It is diagnosed by obtaining a biopsy specimen from a lymph node that shows characteristic histopathologic findings. ⋯ We highlight a 42-year-old Japanese man who presented with persistent fevers and cervical lymphadenopathy who was misdiagnosed at multiple urgent care facilities and emergency departments. He was eventually diagnosed with Kikuchi disease after a biopsy specimen was obtained from a lymph node 1 month after his symptoms began. He was treated with hydroxychloroquine and prednisone with resolution of his symptoms. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: Although Kikuchi disease has been previously described in various specialty medical journals, the published literature on Kikuchi disease in emergency medicine is relatively scarce. Given that cervical lymphadenopathy is a common presentation to emergency departments, it is important for emergency physicians to be cognizant of Kikuchi disease in the differential diagnosis of cervical lymphadenopathy to prevent misdiagnoses and unnecessary treatments.
