Case ReportsRocuronium anaphylaxis in a 3-year-old girl with no previous exposure to neuromuscular blocking agents.
During the perioperative period, anaphylactic reactions rarely occur. Neuromuscular blocking agents (NMBAs) are responsible for 60-70% of perioperative anaphylactic reactions. This case, we report a case of rocuronium-induced anaphylaxis in a 3-year-old girl with no previous exposure to NMBAs. This case cautions and informs practitioners that an IgE-mediated anaphylactic reaction with rocuronium is possible even in young children with no previous exposure to NMBAs.

Clinical features and aggravating factors in nummular eczema in Thais.
Nummular eczema is an idiopathic inflammatory skin disease that mostly has a chronic and relapsing course. Many factors are thought to be responsible for this condition, such as atopic dermatitis, dry skin, emotional stress or seasonal variation. ⋯ Clinical features and possible aggravating factors of nummular eczema were reported. The result confirmed the chronic course of the disease. The nummular eczema affected quality of life of patients and itching caused the impairment.

ReviewStaphylococcus aureus superantigens and their role in eosinophilic nasal polyp disease.
Nasal polyposis is a chronic disease of the upper airways which adversely affects the quality of life of patients. Its pathophysiology is still unclear. ⋯ Moreover, different phenotypes may exist in different ethnic groups of patients. This article will review recent data regarding the type of inflammation, cytokine profiles, involvement of macrophages and dendritic cells, and the impact of various organisms (especially Staphylococcus aureus and its superantigens) and their association with lower airway disease (especially asthma).

Clinical TrialPotent inflammatory cytokine response following lung volume recruitment maneuvers with HFOV in pediatric acute respiratory distress syndrome.
Lung volume recruitment maneuver (LVRM) may improve gas exchange but inflating the lungs to nearly vital capacity may cause further lung injuries. Our aim was to determine the potent inflammatory cytokine response following lung volume recruitment (LVRM) with high frequency oscillator ventilation (HFOV) in pediatric acute respiratory distress syndrome (ARDS). ⋯ There was no significant elevation of potent inflammatory cytokines that may indicate further lung injuries in the majority of our patients. However, there was significant elevation of sICAM-1 levels in non-responders and in those who did not survive that may indicate more lung injuries in these individuals.

Case ReportsToxic epidermal necrolysis-like acute cutaneous lupus erythematosus (TEN-like ACLE) in SLE patients: a report of two cases.
Acute cutaneous lupus erythematosus (ACLE) is a specific lesion in systemic lupus erythematosus (SLE) patients. ACLE can be categorized into localized ACLE, generalized ACLE and toxic epidermal necrolysis-like ACLE. Toxic epidermal necrolysis (TEN) that occurs in SLE patients has been infrequently reviewed. This condition is complicated to diagnose because medication can produce a blistering eruption that resembles vesiculobullous disease in SLE. ⋯ The authors have described two cases of TEN-like ACLE which occurred in the context of systemic involvement of SLE. The cutaneous lesion was gradually progressed, with less mucosal involvement and mainly photodistributed. The authors suggest that the complexity and rarity of this condition could be related to systemic severity of SLE.
