Case ReportsSemitendinosus autograft augmentation after bilateral patellar tendon re-rupture: a case report and technique note.
Acute bilateral patellar tendon rupture is a rare occurrence, especially in young patients in the absence of comorbidities. We describe a case of bilateral patellar tendon re-rupture in a young patient without predisposing factors. Further, we explain a technique for autograft augmented patellar tendon repair with bidirectional fixation using an ipsilateral semitendinosus graft in transosseous patellar and tibia bone tunnels. ⋯ This is the first case describing a new fixation technique after bilateral patellar tendon re-rupture. The use of semitendinosus autograft for reconstruction of the patellar tendon after re-rupture is a viable and effective option.

Wave changes in intraoperative transcranial motor-evoked potentials during posterior decompression and dekyphotic corrective fusion with instrumentation for thoracic ossification of the posterior longitudinal ligament.
A prospective clinical study of amplitudes of intraoperative transcranial motor-evoked potentials (TcMEPs) was performed in patients undergoing surgery for the posterior longitudinal ligament of thoracic spine (T-OPLL). ⋯ AH muscles were particularly useful for functional assessment of corticospinal conduction. High BMI, positive PST, modified McCormick scale IV, and low preoperative JOA score were associated with low rates of amplitude appearance. Amplitude deteriorations occurred throughout surgery, except during rod placement, and speedy rigid rod placement is important.

Reliability of the commonly used classification systems for interprosthetic fractures.
An interprosthetic fracture occurs between a hip and knee arthroplasty. There is currently no universally agreed classification. The aim of this study was to determine the interobserver and intraobserver reliability of the most commonly used interprosthetic fracture classifications. ⋯ This study has demonstrated moderate interobserver reliability and substantial intraobserver reliability for both the Platzer and Pires classifications. This paper would recommend the use of either classification for interprosthetic fractures.

Comparison of radiological and functional outcome of unstable intertrochanteric femur fractures treated using PFN and PFNA-2 in patients with osteoporosis.
Presently, unstable intertrochanteric femur fractures are treated commonly with intramedullary nailing devices. Various designs of intramedullary nail are introduced. The conventional Proximal Femoral Nail has given diverse outcome. Complications have also been noted with this implant. Newer designs like Proximal Femoral Nail Antirotation-2 have been introduced for Asian population. The aim of our study was to compare the radiological and functional outcome of unstable intertrochanteric femur fracture treated with conventional Proximal Femoral Nail and Proximal Femoral Nail Antirotation-2 in osteoporotic patients. ⋯ The functional outcome (p = 0.102) achieved with both the implants was similar. Good functional outcome can be achieved, when the radiological parameters are restored, i.e. TAD < 25 mm, Cleveland index in centre-centre position and neck shaft angle difference < 5°. The overall complications, in the set-up of osteoporosis, seen with both the implants were similar (p = 0.44). PFNA2 group showed better results in terms of perioperative morbidity.

Case ReportsUse of a stainless steel locking calcaneal plate for quadrilateral plate buttress in the treatment of acetabular fractures.
Operative fixation of acetabular fractures involving the quadrilateral surface presents a challenging clinical scenario. Classically, quadrilateral plate buttress was achieved via the use of a "seven" plate. ⋯ The current study presents using a stainless steel locking calcaneal plate to obtain similar surface area coverage as the modern pre-contoured quadrilateral plate, but at a lower cost. Level of evidence IV.
