To assess the value and the associated morbidity of endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic endoprosthesis insertion in the treatment of patients with Klatskin tumors. ⋯ In patients with Klatskin tumors, ERCP and endoscopic endoprosthesis insertion have a low successful drainage rate, are associated with high morbidity and procedure-related mortality, and have a limited effect on long-term palliation. Endoscopic retrograde cholangiopancreatography and endoscopic endoprosthesis insertion have a limited value in the management of patients with Klatskin tumors.

To develop a predictive model identifying perioperative conditions associated with postoperative pulmonary complications (PPCs). ⋯ We identified 3 potentially modifiable risk factors for PPCs. If validated, our results may lead to modifications of perioperative care that will further reduce PPCs.

To identify graduates of medical schools outside of the United States who are on academic general surgical faculties in the United States and to determine their contribution to surgery in the United States. ⋯ International medical graduates account for 10% of the academic surgical faculty in the United States. These graduates come from all countries and regions of the world. International medical graduates occupy faculty positions commensurate with experience. Fourteen are or have been department chairpersons. Since 1960, there has been an increasing number of appointments of IMGs to faculty of the departments of surgery. The results of this survey suggest that the experience of the IMG in US surgery has been quite favorable.

To evaluate the results of a thoracoabdominal hepatectomy and a transdiaphragmatic hepatectomy for hepatocellular carcinoma in patients with impaired liver function. ⋯ The outcomes of the patients undergoing thoracoabdominal hepatectomy and those undergoing a transdiaphragmatic hepatectomy were generally satisfactory in spite of the fact that these procedures were performed on patients with cirrhosis and impaired liver function.
