Laparoscopic hyperthermic intraperitoneal chemotherapy for the management of advanced peritoneal carcinomatosis.
The role of laparoscopic HIPEC has been reported by several centers around the world as a promising and appealing therapeutical alternative in the management of patients suffering of advanced intra-abdominal malignancies and peritoneal carcinomatosis. Laparoscopic HIPEC is implemented as a neoadjuvant and adjuvant therapeutical modality before and after open cytoreductive surgery. The diagnostic and therapeutical impact of laparoscopic HIPEC is well appreciated and generally is associated with many advantages when compared to classical open HIPEC and simultaneous open cytoreductive surgery.

Influence of neutrophil elastase inhibitor on the postoperative course in patients with esophageal cancer after transthoracic esophagectomy.
We evaluated influence of sivelestat sodium hydrate in the clinical course after transthoracic esophagectomy. ⋯ We demonstrated that sivelestat might contribute to the improvement of acute lung injury, hyperbilirubinemia and postoperative circulatory failure.

Comparative StudyBipolar vessel sealing system vs. clamp crushing technique for liver parenchyma transection.
To evaluate the impact of the traditional clamp-crush technique and a radiofrequency bipolar vessel sealing device (BVSD) for liver resection on operative blood loss, transfusion rate, duration of operation, length of hospitalization and morbidity. ⋯ Increased tissue damage in the BSVD group did not seem to correlate with organ dysfunction or postoperative morbidity. The two techniques appear equivalent in term of outcome and thus the choice of transection strategy remains according to the surgeon preference and experience.

Use of band cell percentage as an early predictor of death and ICU admission in severe acute pancreatitis.
Severe acute pancreatitis has a high mortality rate, with over half of the deaths occurring from multiorgan failure in the first seven days of hospitalization. Recruitment of neutrophils to the inflammation in the pancreas is the early event that can escalate to the systemic inflammatory response that causes multiorgan failure. We undertook a retrospective chart review study to ask whether the percentage of immature neutrophils (band cells) in plasma could be used as an early predictor of adverse outcomes (ICU admissions and deaths). ⋯ Band cell percentage might be used clinically as a simple early predictor of adverse outcome in severe acute pancreatitis.

ReviewPeritoneal carcinomatosis cytoreductive surgery and HIPEC: a ray of hope for cure.
This article offers a review on the use of cytoreductive surgery (CS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) for the treatment of peritoneal carcinomatosis (PC). Traditionally, PC was treated with systemic chemotherapy alone with very poor response and a median survival of 6 months or less. ⋯ The extent of intraperitoneal tumor and the completeness of surgery are the most important factors of postoperative outcome. On the other hand, the need of phase III studies, explains a consensus among many surgical oncology experts throughout the world about the use of this new treatment strategy as a standard of care.
