Bisegmentectomies as alternative to right hepatectomy in the treatment of colorectal liver metastases.
Liver surgery for colorectal metastases has moved toward parenchymal sparing strategy with an increasing number of conservative resections. The aim of this study was to elucidate outcomes of bisegmentectomies for right liver metastases. ⋯ Bisegmentectomies are a safe alternative to right hepatectomy in right liver metastases treatment, improving postoperative outcomes, offering similar survival results and increasing the opportunity to re-resection in case of recurrence.

Gasless laparoscopic distal gastrectomy with Roux-en-Y reconstruction.
Laparoscopic-assisted distal gastrectomy (LADG) is a minimally invasive procedure for patients with gastric carcinoma. We have previously reported gasless LADG with a Billroth-I reconstruction using a 5-7cm minilaparotomy. We modified the technique for gasless LADG to include a Roux-en Y reconstruction and to expand the indications for its use. ⋯ Gasless LADG with Roux-en-Y reconstruction is a feasible, novel procedure for a minimally invasive approach to gastric cancer.

The natural history of young patients admitted for acute diverticulitis in terms of the virulence of the disease and the need for surgical treatment has remained controversial. ⋯ Young patients are more often admitted for uncomplicated diverticulitis than older patients. The course of the disease in patients aged less than 50 years is not more aggressive than in older patients.

Case ReportsSpleen preserving laparoscopic distal pancreatectomy with segmental resection of splenic artery in a solid pseudo papillary tumor of the pancreas.
Laparoscopic distal pancreatectomy is suitable for benign and premalignant neoplasms located in the body and tail of the pancreas. In addition, spleen-preservation following distal pancreatectomy for benign or low-grade malignant disease is known to be safe. We present a case of 32-year-old female patient with a solid pseudopapillary tumor of the pancreas treated by spleen-preserving laparoscopic distal pancreatectomy with segmental resection of the splenic artery and splenic vein intact. She had uneventful postoperative recovery and was discharged on the 6th postoperative day. Follow up Doppler-ultrasound scan showed no evidence of splenic infarction and well-preserved splenic blood flow. ⋯ The surgical strategy of segmental resection of the splenic artery with intact splenic vein for spleen preservation seems feasible and represents an alternative option for spleen preservation in laparoscopic distal pancreatectomy. More clinical experience is required to address the advantages, limitations, and reproducibility of this technique.

Randomized Controlled TrialDoes postprandial coffee intake enhance gastric emptying?: a crossover study using continuous real time 13C breath test (BreathID system).
Coffee is one of the most popular beverages worldwide, however, few studies have examined the effects of coffee on the gastrointestinal system. The aim of this study was to determine whether there was a correlation between coffee intake and gastric emptying using a novel non-invasive technique for measuring gastric emptying with a continuous real time 13C breath test (BreathID system: Oridion, Israel). ⋯ The decrease in the T 1/2 and T lag suggests the acceleration of gastric emptying. This study showed that postprandial coffee intake enhances gastric emptying, suggesting the potential use of coffee in clinical settings for patients with functional gastrointestinal disorders.
