Approximately 10% to 15% of cases of ICH involve the thalamus. Evacuation of a thalamic hematoma by craniotomy is associated with high rates of mortality and morbidity. Evacuation by endoscopic surgery is less invasive but is relatively inefficient because of limited visualization of the surgical field. Therefore, a procedure using a polypropylene endoscopic sheath was developed to improve endoscopic visualization and the efficiency of endoscopic evacuation of thalamic hematoma. ⋯ Use of a rigid endoscopic sheath in combination with an endoscope and an approach from Keen's point to the collateral trigone of the lateral ventricle improves the efficiency of evacuating thalamic hematomas and prevents shunt-dependent hydrocephalus.

Effects of intravenous anesthesia with propofol on regional cortical blood flow and intracranial pressure in surgery for moyamoya disease.
The aim of this study was to compare the effects of inhalation anesthesia with sevoflurane and intravenous anesthesia with propofol on ICP and rCoBF during revascularization surgery for patients with MMD. ⋯ Intravenous anesthesia with propofol has potential to provide brain protection and preservation of rCBF in the frontal lobes in surgery for MMD. Whether choice of anesthetic agents might be important in surgery for MMD should be investigated further.

Some patients with MS suffer from disabling tremor. Improvement with medical treatment is modest, at best. Stereotactic surgery targeting the vim nucleus of the thalamus has been successful in alleviating MS tremor. Gamma knife radiosurgery represents a minimally invasive alternative to radiofrequency lesioning and DBS that can provide improvement in patients suffering from essential and parkinsonian tremor. We reviewed our experience with GK thalamotomy in the management of six consecutive patients suffering from disabling MS tremor. ⋯ Gamma knife radiosurgical thalamotomy is effective as a minimally invasive alternative to stereotactic surgery for the palliative treatment of disabling MS tremor.
