The present study describes the use of a limited subtemporal extradural anterior petrosectomy with preoperative embolization of the inferior petrosal sinus for the management of tumors located behind the clivus and ventral to the brainstem. Details of the procedure and its application in five cases are presented. ⋯ Quite easy to master, the anterior petrosectomy with preoperative embolization of the inferior petrosal sinus is a time-conserving approach giving one of the best routes to reach the ventral brainstem while working in front of the cranial nerves and preserving hearing.

Meralgia paresthetica, a syndrome of pain and/or dysesthesia in the anterolateral thigh, is normally caused by an entrapment of the lateral femoral cutaneous nerve (LFCN) at the anterior superior iliac spine. In a few cases compression of the nerve in the retroperitoneum has been reported to mimic meralgia paresthetica. ⋯ In patients with meralgia paresthetica we emphasize a complete radiological investigation of the lumbar spine, including MRI, to exclude radicular compression by a disc herniation or a tumour at the level of L2 or L3.

Surgical resection of a cerebral arteriovenous malformation for treatment of superficial siderosis: case report.
To our knowledge, there are only two reported cases of cerebral arteriovenous malformation associated with superficial siderosis. In both cases, the patients were asymptomatic and were discovered on retrospective review of magnetic resonance imaging. ⋯ We report the first case of surgical resection of an intracranial arteriovenous malformation for the treatment of superficial siderosis. We emphasize the necessity of a detailed evaluation when superficial siderosis is suspected to localize and resect potential bleeding sources, because the disease is progressive and often irreversible.

This case illustrates that although percutaneous subdural tapping for patients with chronic subdural hematoma (CSDH) is successful and minimally invasive, it can be complicated by acute epidural hematoma. ⋯ Hemorrhagic complications after percutaneous subdural tapping are rare. The formation of acute epidural hematoma during twist-drill craniostomy has not been reported in the literature. This complication can occur when the blunt tip of the drill-needle remains on the dura mater without penetrating into the subdural hematoma cavity.

Hemifacial spasm is usually caused by compression of the facial nerve by ipsilateral blood vessels. Compression of the facial nerve root exit zone by a contralateral tortuous vertebral artery is very rare. ⋯ Compression of the facial nerve by the contralateral tortuous vertebral artery may produce hemifacial spasm. A transposed large vessel can be secured by a sling technique or by interposing a soft implant between the brainstem and the vertebral artery.
