Posterior cervical transfacet fusion with facetal spacer for the treatment of single-level cervical radiculopathy: a randomized controlled prospective study.
Single-level cervical radiculopathy may be treated conservatively with cervical tractions. Posterior cervical transfacet fusion with a facetal spacer is a viable option. The aim of the present study is to compare posterior cervical transfacet fusion with conservative physical treatment in single-level cervical radiculopathy. ⋯ posterior cervical transfacet fusion is a safe and effective procedure to treat single-level cervical radiculopathy.

Ruptured Mycotic Aneurysm and Cerebral Vasospasm in the Setting of Endocarditis and Heart Failure Requiring Cardiothoracic Surgery: Case Report and Review.
Ruptured mycotic aneurysm in the setting of cardiac failure and cerebral vasospasm presents unique management challenges. ⋯ Open craniotomy, neuroendovascular treatment, and cardiac surgery strategies can be used when treating patients with ruptured mycotic aneurysms and cardiac failure. When the patient also has cerebral vasospasm, maintenance of mean arterial pressure is paramount.

Meningeal preservation in a child mummy from Ancient Egypt.
Over many centuries, the ancient Egyptians developed a method of preserving bodies so they would remain lifelike. Mummification of bodies was originally a natural process in Egypt, and it evolved to a sophisticated embalming system to preserve the individual for the afterlife. ⋯ One of them was a 6-8 old-year male child with a damaged skull that allowed us to see the meningeal structures covering the entire cranial vault, in absence of brain remains. This finding in a child mummy is exceptional, as reflected in the specialized literature.

The treatment of spinal disorders requires the consideration of a number of factors and understanding the type of material we are implanting is important. Alloys have different mechanical properties and behave differently under different physiologic conditions. ⋯ Several of the important properties that are considered when selecting an alloy for use in spinal instrumentation are explored and detailed for each. This allows for an assessment and comparison of each alloy and a possible determination of which is the best alloy for specific surgery or the best alloy for use in specific situations.

Clivus metastasis of a duodenal adenocarcinoma: a case report and literature review.
Clival metastases of adenocarcinomas are exceptionally rare tumors, especially when they arise from the small intestine. We present the first, to our knowledge, report of a metastasis of a duodenal adenocarcinoma to the clivus. We also present a systematic review detailing metastasis to the clivus. ⋯ Most primary neoplasms originated from the prostate, kidney, and liver, which differ from previous reports on skull base metastases. Abducens nerve palsy is often the first presentation of clival metastasis. Clival metastasis from duodenal carcinoma, although very rare, should be considered in the differential diagnosis of bony lesions of the clivus in a patient with a history of duodenal adenocarcinoma.
