A 69-year-old woman with bilateral breast implants and a newly inserted ventriculoperitoneal shunt presented with a 4-day history of swelling of the right breast. There were no signs of infection and blood tests were normal. ⋯ She was discharged the next day. Ventriculoperitoneal shunt migration to the breast is a rare complication that can lead to cerebrospinal fluid accumulation and may be avoided by carefully tunneling away from the implant.

To analyze the prevalence of retrotransverse foramen (RTF) or retrotransverse groove (RTG) anatomic variations in Chinese atlas vertebra (C1). ⋯ The incidence of RTG is higher than the incidence of RTF. Incidence of bilateral RTF, bilateral RTG, unilateral left RTF, unilateral left RTG, and RTG differed between males and females. Preoperative understanding of these variations using three-dimensional computed tomography angiography is helpful for safe execution of upper cervical posterior approach surgery.

The endoscope-assisted subtemporal key-hole epidural approach (ESKEA) has been recently described. The aim of this study was to measure working volumes and exposure of key areas of the middle cranial fossa provided by this approach. ⋯ ESKEA can be conceived as a modular approach: the 3 surgical corridors have specific working volumes, which are clearly influenced by the degree of temporal lobe retraction, and provide exposure of different middle cranial fossa areas.

Neurosurgery in Pakistan is an advanced field with significant challenges. The training program tests one's endurance, knowledge, empathy, and dedication. The training structure in Pakistan consists of 5 years, of which 2 years are spent in general surgery followed by 3 years of dedicated neurosurgery. This study aimed to conduct a survey to address the quality of neurosurgery training in Pakistan so that its strengths and deficiencies could be identified, recommendations could be made, and actions could be taken toward improvement. ⋯ The neurosurgical training program in Pakistan requires updating of the curriculum with better implementation. These data will help the College of Physicians and Surgeons Pakistan in standardizing neurosurgical training across Pakistan to improve competence among trainees.

We sought to investigate the feasibility of intraoperative local field potential (LFP) recording from the microelectrode during deep brain stimulation surgery for patients with Parkinson disease. ⋯ Despite low signal-to-noise ratio and susceptibility to noises from external sources, this study shows that intraoperative recording of beta LFP using microelectrode is feasible. And, given that no significant difference in peak frequency of beta LFP between intraoperative and postoperative LFP was found, we suggest that not only intraoperative beta LFP can be used as a reliable surrogate for postoperative beta LFP, but it can also provide us an information for estimating the location with maximal power of beta oscillation within the subthalamic nucleus.
