The petrooccipital fissure (POF) has relevance to skull base approaches, various tumors and craniosynostoses, and some cases of age-related hearing loss. However, the prevalence of fusion and classification of such is rarely found in the extant medical literature. ⋯ A POF fusion was relatively common and associated with an ossified petroclival ligament and intrajugular bony septation. Such a prevalence is important for clinicians and skull base surgeons interpreting imaging of the skull base.

The Glasgow Coma Scale (GCS) is intended to be an objective, reliable measure of a patient's mental status. It is included as a metric for trauma registries, having implications for performance metrics and research. Our study compared the GCS recorded in the trauma registry (GCS-1) with that recorded in the neurosurgery consultation (GCS-2). ⋯ The immediate GCS score recorded on patient arrival after trauma differs significantly from the GCS score recorded at later times. This finding significantly altered the probability of survival as calculated by the TRISS methodology. This situation could have profound effects on risk-adjusted benchmarking, assessments of quality of care, and injury severity stratification for research. More studies into the optimal timing of GCS score recording or changes in GCS score and their impact on survival are warranted.

The limitations of anterior cervical discectomy and fusion (ACDF) are related to mechanical failure of the construct after recurring subsidence and migration. In the present study, we evaluated the effect of the maximum rotation of variable angle screws on the range of motion (ROM), cage migration, and subsidence. ⋯ Our results indicate that dynamic plating has promising potential (greater ROM and lower von Mises stress of discs) for stabilization in multilevel anterior cervical discectomy and fusion than static plating, although both dynamic and static plates showed lower ROM than the intact model. A lower screw rotational angle resulted in superior biomechanical performance (lower incidence of migration and subsidence) compared with a higher rotational angle in multilevel applications, regardless of loading.

The Comprehensive Aneurysm Management (CAM) study is a pragmatic trial designed to manage unruptured intracranial aneurysm (UIA) patients within a care research framework. ⋯ CAM was successfully integrated into routine practice. Meaningful conclusions can be obtained if multiple centers actively participate in the trial.

Correctly triaging patients to a surgeon or a nonoperative provider is an important part of the referral process. Clinics typically triage new patients based on simple intake questions. This is time-consuming and does not incorporate objective data. Our goal was to use machine learning to more accurately screen surgical candidates seen in a spine clinic. ⋯ Our models used patient data to accurately predict whether patients will receive a surgical recommendation. The high negative predictive value demonstrates that this approach can reduce the burden of nonsurgical patients in surgery clinic without losing many surgical candidates. This could reduce unnecessary visits for patients, increase the proportion of operative candidates seen by surgeons, and improve quality of patient care.
