Surgical Approach For Suprasellar Meningiomas Based On The Attachment - A Single Surgeon Experience.
Suprasellar meningiomas (SMs) can originate from midline or paramedian dura. Complexity of surgery and visual outcome depend on the attachment of SM. Conventional surgery for SM was ipsilateral to the visual deficit. We report our experience of surgical management of SM after choosing a surgical approach based on the attachment of the SM. ⋯ In patients with SM undergoing surgical resection, opting for an ipsilateral approach when the SM had a midline attachment and a contralateral approach if the SM had a paramedian attachment may ensure greater resection of the lesion and better clinical outcomes. Our findings need validation in larger, randomized studies.

To compare outcomes of proton radiation therapy (PRT), stereotactic radiosurgery (SRS), and x-ray-based radiation with an SRS boost (XRT + SRS) for newly diagnosed clival chordoma. ⋯ Patients after subtotal resection or biopsy may benefit from the incorporation of SRS.

Accurate and on-time confirmation of brain death (BD) is necessary to prevent unnecessary treatment and allow for well-timed organ harvest for transplantation. Although the clinical criteria for BD are legally reliable in some countries, others might prefer complementary ancillary tests to assess the brain's electrical activity and/or blood circulation. The present study aims to define the sensitivity and specificity of computed tomography angiography using 4-, 7-, and 10-point tests compared with the clinical criteria and electroencephalographic findings in patients with BD. ⋯ According to our findings, the CTA-based 4-point scoring system with 100% specificity can be used with the clinical examination findings to confirm BD.

Normal pressure hydrocephalus (NPH) is a frequent disease in elderly patients. The main symptoms are gait disturbance, urine incontinence, and cognitive decline. Fecal urgency and incontinence are described as rare additional symptoms; however, no exact numbers are known. The aim of this study was to investigate the prevalence of fecal disturbances in NPH patients. ⋯ Fecal urgency and incontinence is a frequent finding in NPH (32%) and is essential for the quality of life. In the general population, fecal incontinence in elderly is found in up to 15%. The more than two-fold higher prevalence in NPH patients and the high percentage of postshunted improvement suggests that NPH causes often directly fecal disturbance.

We investigated the indirect puncture method using a novel arc puncture-guided device in percutaneous transforaminal puncture to improve puncture accuracy and reduce the fluoroscopy, puncture, and operation times. ⋯ Indirect puncture using the novel arc puncture-guided device for percutaneous transforaminal puncture can significantly improve puncture accuracy and reduce the fluoroscopy, puncture, and operation times. Indirect puncture using the novel device in percutaneous transforaminal endoscopic discectomy is a potential and practical puncture method.
