A continuous, high flow gas was incorporated into the conventional intermittent mandatory ventilation (IMV) circuit attached to the pressure or volume cycled ventilator which had no built-in IMV mode. These created a continuous flow IMV-CPAP system. During the spontaneous breathing period, the gas flow was high enough to raise a 5-6 cm H2O CPAP level and cause minimal airway pressure fluctuation, an indicator of reduced breathing work. ⋯ During the weaning course arterial pCO2 retained gradually to their steady states in chronic CO2 retaining patients. This resulted in very minimal fluctuations in arterial pH. All patients were weaned successfully with the average weaning duration of 14.19 days.

Case ReportsSpontaneous granulomatous amebic encephalitis: report of four cases from Thailand.
Granulomatous amebic encephalitis (GAE), or meningoencephalitis caused by Acanthamoeba sp and leptomyxid amebae are uncommon CNS infections that usually occur in an immunocompromised host. From 1990 to 1992, 4 patients with GAE were treated at Siriraj Hospital, Bangkok. One case was diagnosed antemortem, from a brain biopsy. ⋯ One patient had a chronic skin ulcer in which free-living amebic trophozoites were found. No visceral involvement was observed. All patients developed "spontaneous" GAE, but we suspect an undiagnosed abnormality in cell mediated immunity or a deficient humoral immune response.

Epidemiologic characteristics of drivers, vehicles, pedestrians and road environments involved in road traffic injuries in rural Thailand.
This report presents descriptive epidemiology of vehicle-related crashes that caused severe injuries in a defined geographical area in northern Thailand, conducted to determine the nature of injuries and factors that contributed to the crashes. In each incident, the driver and other victims were identified and interviewed by a trained public health worker using a standard investigation form. During 1991, 286 single-vehicle crashes (66.4%), 117 crashes with other vehicles (27.1%), and 28 crashes involving pedestrians (6.5%) occurred in the area. ⋯ Poor vehicle conditions and road environments reported in some incidents may have posed additional risks. Pedestrian factors, including young and old age, and alcohol use, may also have contributed to the occurrence of pedestrian injuries. Our system of data collection using an investigation form can provide relevant information, leading to the development of appropriate accident prevention programs for the community.

Does laparoscopy have a limited role in diagnosis of fibroadhesive tuberculous peritonitis?
The information on the limitations of laparoscopy (LP), particularly in the diagnosis of fibroadhesive form of tuberculous peritonitis (TP) is insufficient. Some reports suggested the risk and insufficient information obtained from LP in the diagnosis of fibroadhesive tuberculous peritonitis (FTP). The objectives of this study were to determine the usefulness and limitations of LP in the diagnosis of FTP and ascitic TP. ⋯ No complications or limitations were found. Our study revealed that laparoscopy was a safe, accurate, and uncomplicated method for diagnosis of FTP and ascitic TP. It gave sufficient information, so diagnostic laparotomy was unnecessary.
