Introduction Hypertension is the most common cardiovascular disease (CVD) and a major cause of premature death. Study of age/gender-related and social aspects of the disease, and the assessment of the efficacy of antihypertensive treatment are essential elements of the epidemiological monitoring of hypertension and support a reasonable approach to planning further therapeutic and preventive interventions. Objective Assess the prevalence of hypertension in the working-age population, examine the relationship between hypertension patterns and level of education of respondents taking into account age, gender, and the main aspects of lifestyle. ⋯ On the other hand, patients with higher education had a lower prevalence of hypertension, significantly higher awareness of the disease and efficacy of the treatment than those who had secondary or primary school education. Overall, the study confirmed that the sample of industrial workers could be a reliable source for monitoring hypertension. The significant gender differences and an independent protective effect of the level of education were identified in the epidemiology of hypertension, which should be taken into account in further studies.

In this article we present discussion of the current state of the problem of surgical treatment of ischemic cardiomyopathy (ICM). The pathophysiological aspects of left ventricular remodeling in patients with ICM are also covered. A detailed characterization of methods for assessing the myocardial viability is given and their role in patients with ICM is shown. ⋯ Limitations of the Surgical Treatment for Ischemic Heart Failure (STICH) study are analyzed. The article is intended for cardiologists, general practitioners and cardiac surgeons.

In recent years, a greater understanding of the heterogeneity and complexity of chronic obstructive pulmonary disease (COPD) has come from the point of view of an integrated clinical assessment of severity, pathophysiology, and the relationship with other pathologies. A typical COPD patient suffers on average 4 or more concomitant diseases and every day about a third of patients take from 5 to 10 different drugs. The mechanisms of the interaction of COPD and cardiovascular disease (CVD) include the effects of systemic inflammation, hyperinflation (hyperinflation) of the lungs and bronchial obstruction. ⋯ The prevalence of coronary heart disease, heart failure, and rhythm disturbances among COPD patients is significantly higher than in the general population. The article discusses in detail the safety of prescribing various groups of drugs for the treatment of CVD in patients with COPD. Achieving success in understanding and managing patients with COPD and CVD is possible using an integrated multidisciplinary approach.

to establish associations of candidate genes ACE, AGT, AGTR1, ADRB1, ADRA2B, MTHFR and NOS3 polymorphisms with left ventricular myocardial hypertrophy (LVH) in patients with arterial hypertension (AH) among the indigenous (the Shors) and non-indigenous population of Mountain Shoria. ⋯ The following studied genes were found to be associated of with LVH: in the Shor cohort - the MTHFR gene (log additive model of inheritance), the A/G genotype of the ADRB1 gene (among people with normal body weight), the I/D genotype of the ACE gene (among men); in the non-indigenous cohort - D/D genotype of the ACE gene (the codominant model of inheritance), the NOS3 gene (the log additive model of inheritance).

The article contains an outline of the 2017 ACC Expert Consensus Decision Pathway for Periprocedural Management of Anticoagulation in Patients With Nonvalvular Atrial Fibrillation. This document considers in detail problems of necessity and safety of temporary interrupting of anticoagulation for the period of procedure, main principles of interruption and restarting anticoagulant therapy after procedure, indications to the transitional (bridging) therapy in the periprocedural period, as well as possible strategies of periprocedural management of patients in dependence of risk of bleeding and thromboembolic complications. This Expert Consensus Decision Pathway refer to the periprocedural use of both oral (vitamin K antagonists, new oral anticoagulants) and parenteral (unfractionated and low-molecular-weight heparins) anticoagulants.
