Electrocardiographic characteristics of abnormal left coronary arterial branching (ALCAB) from the pulmonary trunk were examined in 36 patients, aged 2 months to 39 years. Signs of marked left-ventricular and left-atrial overstrain were typical for all ages. There were signs of past left ventricular myocardial infarction in 88.8% of patients, the cicatricial changes being anterolateral in 65.6%, which is typical for the defect. The follow-up of ECG pattern over 1 month to 8 years after the defect was first detected demonstrated that coronary circulation deteriorated in 64.3% of cases.

Comparative Study
[Fatal outcome of acute myocardial infarction in men and women].
An epidemiologic study of the outcomes of acute myocardial infarction, carried out according to the WHO Register of Acute Myocardial Infarction, demonstrated that overall mortality rates are similar for men and women between 20 and 69 years of age. Prehospital mortality was significantly higher in men, as compared to women, while the opposite was true for hospital mortality. Overall, prehospital and hospital mortality rates were relatively high in the younger patients, both male and female, an evidence of a more severe course of acute myocardial infarction at a younger age.

Case Reports Historical Article
[Evolution and current problems in reconstructive surgery of the heart valves].
Progress of valvular cardio-surgery is reviewed. Advantages and shortcomings of mitral commissurotomy are discussed; to-day, the operation falls short of providing the hemodynamic effect achievable by using up-to-date valvular prosthetic appliances. ⋯ The evolution of artificial valve prostheses is reviewed; good results with the use of biological calf xenopericardium prostheses are mentioned. The use of artificial and biological prostheses can at present provide better opportunities for the correction of valvular defects of the heart.
