[Asymmetric pulmonary perfusion causing unilateral pulmonary edema as a complication of acute myocardial infarction].
Acute unilateral pulmonary edema is an unusual clinical condition occasionally associated with severe mitral valve insufficiency. We describe a patient diagnosed as having unilateral pulmonary edema after an acute anterior myocardial infarction. ⋯ The diagnosis of acquired hypoplasia of the left pulmonary artery tree was made by chest computed tomography. The pathogenesis and differential diagnosis are discussed.

The nature and outcome of treatment for acute myocardial infarction in elderly patients admitted to Spanish hospitals with primary angioplasty facilities are not well documented. ⋯ Over 42% of elderly patients with myocardial infarction admitted to Spanish hospitals with angioplasty facilities did not receive reperfusion therapy. Thrombolysis was the most frequently used reperfusion therapy. However, neither thrombolysis nor primary angioplasty improved 30-day mortality.

[Clinical, echocardiographic and prognostic profile of Streptococcus viridans left-sided endocarditis].
Published case series on Streptococcus viridans endocarditis are scarce and outdated. The aims of our study were multiple: to analyze the profile of the Streptococcus viridans endocarditis, to compare it with other types of left-sided endocarditis and with cases caused by Staphylococcus aureus, and to determine predictors of poor outcome in Streptococcus viridans endocarditis. ⋯ Although Streptococcus viridans is a nonaggressive microorganism, valvular destruction is similar to that caused by other pathogens when it causes left-sided endocarditis. Nonetheless its prognosis is better, a feature which may be related to the fact that the systemic infectious syndrome can be treated more effectively. Prognostic factors in left-sided endocarditis due to Streptococcus viridans are heart failure and periannular complications.

Pneumopericardium is an uncommon but potentially serious condition. It appears in association with invasive diagnostic and therapeutic procedures, although cases have also been reported in association with bronchospasm, esophageal disorders, and other problems. We describe a case of spontaneous pneumopericardium with chest pain in a patient with community-acquired pneumonia.
