This is a report on the use of a stapling device for caesarean sections. The device can be inserted right and left after a very small median uterotomy. ⋯ Haemostasis is definitely assured. No adverse reactions or disadvantages for the patients have been noticed to date.

Comparative Study[Length of the cervix in the 2d and 3d trimester: vaginal study versus measurement by perineal scan--an improved indication guide for cerclage?].
In a study made at the Department of Gynaecology of the University of Heidelberg, the cervical length, as measured by vaginal examination, was compared in 100 impatients between the 16th and 40th week of pregnancy with a special ultrasonic examination method termed "perineal scan". Examination results can be considerably simplified, corrected and, above all, increased in objectivity through the use of perineal scan. As a result, in 53 patients in which, clinically, a definite shortening of the cervix was apparent (portio-length 0.5-1 cm), 12 cases (23%) were found by sonography to exhibit a normal finding with a cervical length greater than or equal to 3 cm. ⋯ The superior objectivity of the results obtained by perineal scan when compared to vaginal examination could be statistically established, primarily through the number of premature births occurring during the further course of pregnancy. The incidence of premature births was not significantly increased in clinically established cervical shortening of less than 1.5 cm, whereas a significant result was obtained if sonography revealed cervical shortening below 2.5 cm (p less than 0.01). Perineal scan is however also recommended for controlling therapeutic success of tocolytic treatment and for confirming and/or detecting premature rupture of the foetal membranes, as well as in cases of suspected cervical insufficiency or incompetent cervix.(ABSTRACT TRUNCATED AT 250 WORDS)

[Assessment by sonography and palpation of the cervical closure system].
Sonography or palpation alone are inadequate procedures in evaluating cervical function. Only by combining both methods cervical incompetence can be detected at an early stage, adequate therapy instituted and unnecessary cerclage operations prevented. The value of sonography lies in the possibility of studying the functional changes of the cervix in premature labour.

[Obstetric-perinatologic data collection using a personal computer].
A concept covering the collecting and processing of obstetrical and perinatological data is described. Collecting of patient data is effected on the basis of a case history that has been drawn up in an EDP-adequate manner, which, however, can also be used in the conventional way. This procedure was chosen because for some time to come one cannot do without a document for handwritten notes to avoid duplication of work by "double tracking" and to eliminate transmission errors, and also to continue the present procedure of dealing with the patient. ⋯ Besides compiling the usual statistical analyses and formulating research problems, the system automatically prepares the discharge reports. This rationalisation procedure compels the user to collect the data with care and also completely. On the whole, such a data collection system offers to hospitals of any size quick and easy access to data at any time, as well as optimised patient care, without additional effort and at a reasonable cost level.

Clinical Trial[Clinical experience with Diane-35, currently the lowest dose antiandrogenic hormonal ovulation inhibitor, in mild to moderative androgenization in the female].
Low-dose hormonal contraceptives with specifically reduced estrogen are now the drug of choice for the prevention of internal risks. The antiandrogenic hormonal contraceptives have also had to be adapted to this situation. ⋯ In the 12th treatment cycle the treatment success rate was almost 90%. The side effects occurring with higher-dose hormonal contraceptives were reduced, cycle behavior was acceptable, and the incidence of metrorrhagia corresponded to that found with other micropills.
