During a period of 3.5 years 13,214 patients were examined in a specialised breast clinic; 87% of the women had been referred by gynaecologists. 73% of the 160 carcinomas discovered 65% of the 143 fibroadenomas and 32% of the 982 cysts were palpable. Among 2,355 patients with a positive palpation, a carcinoma was found only in 5% of cases, whilst a carcinoma was found in 0.4% of the 10,819 patients with negative palpation. Therefore the use of apparatus in breast diagnosis is highly recommended both to avoid unnecessary surgery and to discover small cancers.

Update computer technology is an increasingly useful tool in medicine for more accurate documentation, avoidance of multiple documentation, improvement of service performance, quicker and easier data processing and evaluation and hence quality assurance. We developed an obstetric data recording programme for an IBM compatible personal computer according to the MS-DOS command programme. A laser printer with dual paper feed is available. ⋯ To ensure best possible motivation and acceptance, special attention was paid to user friendliness and immediate print-outs whenever required. Attention was also given to the inclusion of infant follow-up data. The computer programme has been accepted for daily operation, and the necessary data quality has been attained by means of proper choice of the appropriate user groups and manifold measures to ensure integration, safe working, continual servicing and creation of suitable working and data feed conditions.

Extracorporeal life support with membrane oxygenation (ECMO) has been successfully introduced into the management of asphyctic neonates. Two case studies are given to show the importance of this method in antenatal obstetric management.

[Diagnostic results and their clinical interpretation--or: what you never wanted to know about diagnosis].
Diagnostic test results become only relevant if clinically interpreted. A key issue in clinical interpretation is the predictive value which, however, in daily practice and even in medical literature, is frequently misunderstood, as if the predictive value would be about the same as sensitivity. Sensitivity and specificity of a test only together with the prevalence of the target disease allow estimation of the predictive values. ⋯ Finally, considering these correlations, decision analysis using the "threshold model" shows how to develop a rational, quantitative approach to the selection and interpretation of diagnostic tests. Although more and more studies on decision analysis in diagnostics are published, up to now, there is little response in day-to-day practice. This situation should be changed for the sake of our patients.

From 1983-1989, 106 patients with breast cancer were treated in our pain management unit on 6767 treatment days. Pain was caused by bone metastasis in 73% of patients. Neuropathic pain was reported by 32% of the patients. ⋯ Whilst 92% of patients reported more than moderate pain on admission, 45% obtained complete pain relief beginning from the first days of treatment. On 92% of the days, patients described their pain as moderate or less. Side effects were treated symptomatically and played a minor role in a reason to change therapy.
