Parents' perspectives regarding a physician-parent conference after their child's death in the pediatric intensive care unit.
To investigate parents' perspectives on the desirability, content, and conditions of a physician-parent conference after their child's death in the pediatric intensive care unit (PICU). ⋯ Many bereaved parents want to meet with the intensive care physician after their child's death. Parents seek to gain information and emotional support, and to give feedback about their PICU experience.

Randomized Controlled Trial Multicenter StudyPalivizumab prophylaxis, respiratory syncytial virus, and subsequent recurrent wheezing.
Children who experience respiratory syncytial virus (RSV) lower respiratory tract infections (LRTIs) early in life have high rates of subsequent recurrent wheezing. Palivizumab, an anti-RSV monoclonal antibody, has 78% to 80% efficacy in preventing RSV hospitalization in premature infants without chronic lung disease. We hypothesized that palivizumab, by ameliorating or preventing early RSV LRTI in preterm infants, might decrease later recurrent wheezing. ⋯ Our study suggests that preventing RSV LRTI with palivizumab may reduce subsequent recurrent wheezing in premature infants.

Changes in myocardial function and hemodynamics after ligation of the ductus arteriosus in preterm infants.
To characterize the changes in systemic hemodynamics and systolic, diastolic, and global myocardial performance after patent ductus arteriosus (PDA) ligation in very-low-birth-weight infants. ⋯ After PDA ligation, LV output and MPI decrease, due primarily to a decrease in LV preload, although LV contractility and diastolic function do not change. However, the changes in LV MPI after ligation also reflect an acute deterioration followed by an improvement in global cardiac function, because LV loading conditions remained unchanged after surgery and thus cannot explain the improvement in MPI by 24 hours after ligation.

Unexplained neonatal respiratory distress due to congenital surfactant deficiency.
Genetic abnormalities of pulmonary surfactant were identified by DNA sequence analysis in 14 (12 full-term, 2 preterm) of 17 newborn infants with fatal respiratory distress of unknown etiology. Deficiency of adenosine triphosphate-binding cassette protein, member A3 (n = 12) was a more frequent cause of this phenotype than deficiency of surfactant protein B (n = 2).

Randomized Controlled Trial Comparative StudyNasal intermittent mandatory ventilation versus nasal continuous positive airway pressure for respiratory distress syndrome: a randomized, controlled, prospective study.
To evaluate whether nasal intermittent mandatory ventilation (NIMV) compared with nasal continuous positive airway pressure (NCPAP) would decrease the requirement for endotracheal ventilation in the treatment of respiratory distress syndrome (RDS) in preterm infants <35 weeks. ⋯ NIMV compared with NCPAP decreased the requirement for endotracheal ventilation in premature infants with RDS. This was associated with a decreased incidence of BPD.
