The ethics of neonatal resuscitation at the margins of viability: informed consent and outcomes.
To determine the adequacy of records of parental counseling in mothers with threatened preterm delivery before 27 weeks gestation, whether interventions performed at birth were consistent with recorded antenatal decisions and whether extent of resuscitation affected the occurrence of serious short-term morbidity. ⋯ Records of antenatal consultations were often lacking important information. Variations in physician documentation practices are substantial and affect the care offered to infants at the threshold of viability. Even extensive resuscitation can be followed by intact survival if the resuscitation required is brief.

Case ReportsColorimetric end-tidal carbon dioxide detectors in the delivery room: strengths and limitations. A case report.
Clinical assessment and end-tidal carbon dioxide (ETCO2) detectors are used to verify tracheal intubation in newborn infants. A case is presented in which an ETCO2 detector was misleading in determining endotracheal tube (ETT) position but useful in determining the efficacy of ventilation in an extremely preterm infant.

Regional variation in ICU care for pediatric patients with asthma.
To determine adherence to guidelines for severe asthma care and evaluate regional variability in practice among pediatric intensive care units (PICU). ⋯ Adherence to national guidelines for use of inhaled anticholinergics among critically ill children is low, and marked variation in use of invasive ventilation exists. More explicit guidelines regarding indications for invasive ventilation may improve asthma care.

Overweight and obesity in deltaF508 homozygous cystic fibrosis.
Overweight (9%) and obesity (1%) in patients with cystic fibrosis homozygous for the deltaF508 mutation (CFdeltaF508) were non-trivial. Children with CFdeltaF508, in contrast to the general population, showed a positive association between body mass index and lung function for all body mass index z-scores.
