Randomized Controlled Trial Multicenter Study Comparative Study Clinical TrialA multicenter randomized, masked comparison trial of natural versus synthetic surfactant for the treatment of respiratory distress syndrome.
To compare the efficacy and safety of two surfactant preparations in the treatment of respiratory distress syndrome (RDS). ⋯ Compared with Exosurf, Infasurf provided more effective therapy for RDS as assessed by significant reductions in the severity of respiratory disease and in the incidence of air leak complications.

Multicenter Study Comparative StudyEffectiveness of ribavirin in otherwise well infants with respiratory syncytial virus-associated respiratory failure. Pediatric Critical Study Group.
To examine ribavirin's effectiveness in otherwise well infants with respiratory syncytial virus (RSV)-associated respiratory failure. ⋯ Ribavirin administration during mechanical ventilation to previously well infants with RSV infection was not associated with reductions in either mortality rates or duration of mechanical ventilation. Additional clinical effectiveness studies are required to define specific groups in which the use of aerosolized ribavirin is indicated.

Multicenter Study Comparative StudyLow-molecular-weight heparin in pediatric patients with thrombotic disease: a dose finding study.
To compare low-molecular-weight preparations of heparin (LMWH) with standard heparin in children requiring anticoagulant treatment for thromboembolic disease. ⋯ These results provide the basis for a randomized, controlled trial comparing LMWH with standard heparin in pediatric patients.

Case ReportsReiter syndrome initially misdiagnosed as Kawasaki disease.
A misdiagnosis of Kawasaki disease was made initially for two patients with Reiter syndrome. The first patient had conjunctivitis, urethritis, arthritis, and the characteristic skin finding of keratoderma blennorrhagicum. ⋯ Neither patient responded to intravenous immunoglobulin therapy but both responded to nonsteroidal antiinflammatory medication. The clinical characteristics of Reiter syndrome and Kawasaki disease in children are similar but specific features should allow for their differentiation.

Case ReportsExtracorporeal membrane oxygenation as a bridge to definitive tracheal surgery in children.
Extracorporeal membrane oxygenation was used as a bridge for three infants with complicated long segment congenital tracheal stenosis to tracheal homograft transplantation with cadaveric tracheal homograft and for one child, with an extensive traumatic tracheal laceration caused by aspiration of a sharp foreign body, to definitive tracheal repair. In all four cases mechanical ventilation was impossible and death almost certain without extracorporeal membrane oxygenation.
