Tuberculosis of the endometrium is usually associated with infertility. Once a patient is diagnosed with endometrial tuberculosis, she is almost certain to experience infertility; in vitro fertilization and embryo transfer offer the only realistic treatment for tuberculous infertility. We describe one case of unassisted conception with a normal pregnancy outcome in a patient with Mycobacterium tuberculosis infection of the endometrium. ⋯ Despite the successful fertility outcome in our patient, we do not think that this case was a challenge to previous concepts of endometrial tuberculosis. In vitro fertilization and embryo transfer are usually the most suitable options for these patients.

Bilateral vertebral artery dissection causing a cerebrovascular accident in pregnancy. A case report.
Vertebral artery dissection, occurring spontaneously or following a traumatic event, is a cause of posterior circulation stroke in young individuals, including pregnant women. ⋯ Vascular dissections occur rarely during pregnancy. Spontaneous extracranial vertebral artery dissection itself is very rare in general. Cerebral ischemia can follow vertebral artery dissection. In young patients with CVA, consideration of the diagnosis of vertebral artery dissection followed-by angiography and anticoagulation is an important component of the workup and care.

To determine if decreasing the number of prenatal visits for routine obstetric patients affects pregnancy outcome. ⋯ Prenatal visits can be decreased in a teaching hospital in women with uncomplicated pregnancies from the standard number, 12-14 visits, to an average of 7 or 8 per patient without adverse perinatal outcomes.

We studied the effect of endothelin (ET)-1 on spontaneous contractions and the effects of nimodipine and isradipine on ET-1-induced contractions in myometrial tissue from normal and diabetic pregnant women. ⋯ Gestational diabetes increases ET-1-induced contractile response in human myometrium. The contractile effect of ET-1 in normal and diabetic myometrium is mediated partly by dihydropyridine-sensitive calcium channels since it is significantly reduced by nimodipine and isradipine. The promising data from this study warrant clinical studies on the definitive place of nimodipine and isradipine in the treatment of preterm labor, especially in a diabetic woman, to avoid metabolic complications of beta-mimetic tocolytics.

Postmenopausal bleeding is more prevalent than was previously thought, occurring in women regardless of whether or not they are on hormone replacement therapy. While estrogen-only regimens have been used for several beneficial effects, bleeding patterns associated with these regimens can be irregular and unpredictable, causing discomfort to the patient as well as increasing the risk of both endometrial hyperplasia and carcinoma. Studies in recent years have examined the effects of estrogen-only regimens as compared to different estrogen-plus-progestogen combination therapies to help regulate and minimize postmenopausal bleeding while providing endometrial protection.
