Editorial Clinical TrialEfficient prematurity prevention is possible by pH-self measurement and immediate therapy of threatening ascending infection.
In two prospective investigations, the effectiveness of the self care programme for prematurity prevention, developed by Saling, was investigated. Pregnant women in Erfurt have been offered to perform self measurements of their vaginal pH by means of test gloves twice a week in order to screen for any disturbances in the vaginal milieu. The women were instructed to see their physician immediately, if abnormal pH > or = 4.7 or other risk factors were present, in order to get them confirmed and to start lactobacillus acidophilus therapy or in case of bacterial vaginosis to treat with clindamycin cream i. vag. ⋯ On this basis, a significant reduction of early prematurity from 1.58 to 0.99% was seen, respectively (P < 0.001). Comparing low birthweights a significant reduction of cases was achieved as well in all groups. On the basis of the data obtained we recommend the extension of the campaign in whole of Germany.

To establish the extent of maternal mortality in Italy in between 1980 and 1996 in order to compare it with the international data. ⋯ Italian data appear reassuring and encourage further investigations on detailed welfare problems.

To review the incidence, indications, risk factors, and complications associated with emergency peripartum hysterectomy. ⋯ Peripartum hysterectomy is a necessary life saving operation. Timely operation minimizes the morbidity and mortality.

Comparative StudyThe decision to delivery interval in emergency and non-urgent cesarean sections.
The aim of this study was to assess the decision to delivery interval (DDI) in our obstetric unit in comparison to current recommendations. ⋯ The proposed 30 min DDI standard was achieved in 100, 71, 47 and 35% of emergent-crash, emergent, emergent-non-crash and non-urgent cesareans sections, respectively.

Comparison of the effects of nimesulide and 5,5-dimethyl-3-(3-fluorophenyl)-4-(4-methylsulphonyl) phenyl-2(5H)-furanone (DFU) on contractions of isolated pregnant human myometrium.
To compare the effects of 5,5-dimethyl-3-(3-fluorophenyl)-4-(4-methylsulphonyl) phenyl-2(5H)-furanone (DFU) and nimesulide, selective COX-2 inhibitors, on the amplitude and frequency of KCl-, oxytocin-, and PGF(2alpha)-stimulated contractions of isolated pregnant human myometrial strips. ⋯ DFU is a more potent inhibitor than nimesulide on KCl-, oxytocin-, and PGF(2alpha)-stimulated contractions of pregnant human myometrium. The inhibitor effects of nimesulide and DFU were predominantly on KCl-stimulated contractions.
