Case Reports Comparative StudyChemical hypophysectomy for relief of bone pain in carcinoma of the prostate.
A series is presented of 10 patients with intractable pain secondary to diffuse metastatic prostatic carcinoma. These patients underwent pituitary ablation by a new method of chemical hypophysectomy, which consisted of multiple injections of absolute alcohol into the pituitary gland under stereotaxic control. ⋯ The sequelae are those associated with pituitary destruction, the most significant being diabetes insipidus. Preoperative and postoperative hormonal studies failed to reveal any significant hormonal changes despite good and almost immediate pain relief.

Case ReportsFactor XI deficiency: detection and management during urological surgery.
Hereditary factor XI deficiency may remain undiagnosed until severe bleeding is observed after an operation or trauma. Two such cases were encountered and, therefore, a regular screening test for coagulation disorders among urological patients was initiated. During 2 years (1975 and 1976) 10 additional cases of factor XI deficiency were detected: 5 severe and 5 partial. ⋯ In 1 of 3 patients who did not receive transfusions postoperative bleeding was observed. A minimal level of 0.3 U./ml. (30 per cent) factor XI was found necessary to ensure good hemostasis during and after an operation. In view of a recent finding of relatively high gene frequency of factor XI deficiency in Ashkenazic Jews it seems warranted to do a partial thromboplastin time in such patients who need an operation.

The use of microfibrillar collagen hemostat for control of renal bleeding.
Microfibrillar collagen hemostat was used in 23 patients with renal injuries owing to multiple causes in an attempt to control small vessel bleeding from raw renal surface areas. Minimal complications have occurred with the use of this substance. Control of bleeding has been prompt and no late bleeding has occurred.

A penile problem that physicians are confronted with in the emergency room is entrapment of the foreskin by a zipper fastener. A case presentation and a simple technique for its management are described.

Case ReportsAppendicitis simulating torsion of the spermatic cord: a case report.
A case of acute non-perforated appendicitis presenting as acute scrotal swelling and erythema is reported. A communicating hydrocele containing serous cloudy fluid was encountered on examination, suggesting the intra-abdominal pathology.
