To describe the unmet need for pediatric organs, the history of donation after cardiac death (DCD), implementation of DCD policies in children's hospitals, and the current U.S. experience with DCD in children. ⋯ The widening gap between the need for organs and the availability of organs from brain-dead donors has led to a resurgence of both interest in and use of organs donated after cardiac death. Children's hospitals need to explore DCD as an option in select circumstances to serve grieving families who would like to donate and to increase organ availability for transplantation. DCD programs are dependent on input and support from critical care providers.

Comparative StudyValidation of pediatric index of mortality 2 (PIM2) in a single pediatric intensive care unit of Argentina.
Pediatric Index of Mortality 2 (PIM2) is an up-to-date mortality prediction model in the public domain that has not yet been widely validated. We aimed to evaluate this score in the population of patients admitted to our pediatric intensive care unit. ⋯ PIM2 showed an adequate discrimination between death and survival and a poor calibration assessed by the Hosmer-Lemeshow goodness-of-fit test. The standardized mortality ratio and clinical analysis of the Hosmer-Lemeshow table make us consider that PIM2 reasonably predicted the outcome of our patients.

Case Reports Comparative StudySecondary abdominal compartment syndrome in a case of pediatric trauma shock resuscitation.
To report a rare case of secondary abdominal compartment syndrome during shock resuscitation in a pediatric trauma patient. ⋯ Secondary abdominal compartment syndrome developed in this pediatric trauma patient with hemorrhagic shock, possibly from aggressive crystalloid resuscitation. This trauma case highlights the importance of early hemorrhagic control with balanced crystalloid/transfusion therapy. Secondary abdominal compartment syndrome in pediatric trauma is rare and may reflect physiologic differences during development, less aggressive resuscitation practices, and/or underrecognition.

Comparative StudyRetrospective analysis of the prognostic value of electroencephalography patterns obtained in pediatric in-hospital cardiac arrest survivors during three years.
To test the hypothesis that electroencephalography has prognostic value in children after in-hospital cardiac arrest. ⋯ In a single-center consecutive case series, electroencephalography background patterns during the initial 7 days after in-hospital cardiac arrest were associated with neurologic outcome in children.

Case Reports Comparative StudyUse of levosimendan, a new inodilator, for postoperative myocardial stunning in a premature neonate.
To first report the successful use of the new inodilator levosimendan in a premature infant with congestive heart failure (CHF) following cardiac surgery. Although the calcium sensitizer levosimendan improves hemodynamics in adults with CHF, no data are available on the use of levosimendan in premature infants with CHF. ⋯ In this premature neonate with postoperative low cardiac output syndrome due to failing myocardial function, levosimendan was a potent inotropic agent.
