Arterial blood gas analysis is the gold standard for assessing the adequacy of ventilation. However, arterial blood sampling may be associated with serious complications in neonates. The aim of the study was to utilize the side-stream capnometry measurement of end-tidal carbon dioxide (PetCO₂) via nasal cannula circuits and to verify the reliability of PetCO₂ in reflecting the arterial blood carbon dioxide(PaCO₂) level in nonintubated neonates. ⋯ End-tidal CO₂ measurement by side-stream capnometry through nasal cannula could provide an accurate and noninvasive estimate of PaCO₂ levels in nonintubated neonates.

Infected percutaneously inserted central venous catheters (PICCs) are a problem in hospitalized patients, especially in the neonatal intensive care unit. The objective of this study was to assess the risk of infection and other PICC-associated complications in very low birth weight infants. ⋯ It is important to avoid inserting a PICC at the femoral site. Strict catheter care protocol should also be applied to reduce local site bacterial colonization and removal of PICCs as soon as they are no longer essential for patient care to reduce the incidence of infection.

Is capnometry monitoring useful in nonintubated neonates?

We report a full-term female neonate who presented with respiratory distress and severe heart failure soon after birth. Heart failure secondary to perinatal infection was initially suspected. Subsequent echocardiography revealed aortic runoff, which led to consideration of an intracranial vascular abnormality. ⋯ Endovascular coil embolization of the vascular anomaly was performed, resulting in improvement of heart failure. VGAM should be considered in the differential diagnosis of neonatal congestive heart failure with a structurally normal heart. Urgent endovascular embolization and aggressive medical treatment of heart failure improve prognosis in neonatal VGAM.

Effects of flow rate on delivery of bubble continuous positive airway pressure in an in vitro model.
There has been concern over the effect of vigorous bubbling on the delivery pressure during the operation of the bubble nasal continuous positive airway pressure (CPAP) system. We investigated the relationship between intra-tubing pressure changes and flow rates in a closed bubble CPAP system in vitro. ⋯ These results demonstrated that the intra-tubing pressure in a bubble CPAP system was highly correlated with flow rate in vitro.
