Reversible hyperpigmentation of skin and nails with white hair due to vitamin B12 deficiency.
A case of reversible generalized hyperpigmentation of the skin and nails with reversible premature gray hair due to vitamin B12 deficiency is reported. The cause of the vitamin B12 deficiency in this patient was pernicious anemia. The pigmentation of his skin and hair returned to normal after treatment with intramuscular cyanocobalamin.

Five patients with severe drug-induced toxic epidermal necrolysis improved rapidly after one to two plasma exchanges. The improvement of all five patients treated with plasmapheresis contrasts with the disease's mortality rate of up to 50%, as reported in the literature and as observed among our previously treated patients. Since there is no effective treatment for toxic epidermal necrolysis, a controlled clinical trial to evaluate the effectiveness of plasma exchange would seem worthwhile.

A case of botfly myiasis occurred in a patient who traveled to Brazil. This infestation resembles a pyogenic furuncle, but should be a diagnostic consideration when it does not respond to antibiotic therapy in a world traveler.

A patient whose condition was initially misdiagnosed as Kaposi's sarcoma involving the left foot received radiation therapy for that disorder. Subsequent examination yielded a correct diagnosis of arteriovenous (AV) malformation. ⋯ The clinicopathologic distinctions between Kaposi's sarcoma and pseudo-Kaposi's lesions are discussed. All patients suspected of having Kaposi's sarcoma, especially those younger than 30 years of age, should have careful evaluation for an unsuspected AV malformation.

A case of cutis verticis gyrata (CVG) occurred in a patient with no known associated disorders. Under the present classification system, patients with primary CVG are all grouped together. We propose subclassifying patients with primary CVG into two groups: those with associated neurologic and ophthalmologic diseases and those without. Our patient was treated with a scalp reduction procedure.
