Metformin is the first-line therapy for the treatment of diabetes mellitus. In certain conditions lactic acidosis (MALA) can occur. Starting with a case report of a 62-year-old woman presenting with abdominal pain, we bring this complication to attention, describing its pathogenesis and its management. This underlines the need for thoughtful use of metformin.

The prognosis of multiple myeloma patients has significantly improved since the introduction of the novel agents thalidomide, bortezomib and lenalidomide. We report the data of a medical need programme with lenalidomide plus dexamethasone, conducted in Belgium between August 2007 and March 2008, and including 98 relapsed refractory multiple myeloma patients. In addition to chemotherapy and steroids, all patients had received prior treatment with bortezomib, and 84% of them had been exposed to thalidomide. ⋯ Treatment with lenalidomide plus dexamethasone did prolong overall survival by nearly half a year in this population with end-stage myeloma. Overall response and quality of response were independent of previous response to thalidomide and bortezomib, although the time to progression tended to be shorter in thalidomide- and bortezomib-refractory patients. It can be concluded that lenalidomide plus dexamethasone is an effective and safe treatment regimen in highly refractory multiple myeloma patients, and that these responses are irrespective of previous exposure or sensitivity to thalidomide and bortezomib.

Collision in the colon: concurrent adenocarcinoma and diffuse large B-cell lymphoma in the same tumour.
Double malignancies in the gastrointestinal tract are unusual. Concurrent lymphoma and adenocarcinoma is a rare clinical condition. We herein report a collision tumour which first presented with a diffuse large B-cell lymphoma in the skull base and ileocecal junction area. ⋯ Therefore, both lymphoma and carcinoma may be EBV related. Our experience illustrated that such collision tumours are rare and difficult in diagnosis. Clinicians and pathologists should be aware of such an association in order to make a correct diagnosis and initiate proper treatment.

Since 2004, the US Food And Drug Administration and the European Medicine Agency are giving warnings about cocaine adulterated with levamisole. Levamisole is primarily used as an anti-helminthic. One of the side-effects of levamisole is a decreased bone marrow function. Herein we describe the first case of agranulocytosis and neutropenic fever due to cocaine adulterated with levamisole reported in Europe.

Pain acceptance is considered important for mental well-being with better functional outcomes for chronic pain patients. The present study explored whether pain-related variables (pain severity, pain interference, pain duration, and pain catastrophizing) and non-pain-related variables (personality traits) influence acceptance and additionally examined the interrelationship between the influencing variables and acceptance. ⋯ The findings indicate that acceptance is influenced by catastrophizing and avoidant personality traits. The clinical implication might be that acceptance-oriented treatments may prove less successful in chronic pain patients with more pronounced avoidant personality traits. Extra focus on a reduction of the frequency of pain catastrophizing might be helpful.
