Monitoring of neuromuscular transmission by electromyography during anaesthesia. A comparison with mechanomyography in cat and man.
In this study the applicability of the electromyographical method for monitoring neuromuscular transmission during anaesthesia has been investigated. The purpose was to determine the stability and temperature dependence of the EMG and to evaluate and compare EMG area, amplitude and duration during a non-depolarizing neuromuscular block in the tibialis anterior muscle preparation of the cat. Further, in clinical studies the purpose was to investigate differential hand muscle sensitivity to non-depolarizing neuromuscular blocking agents based on the EMG and the standard adductor pollicis mechanomyogram, and to establish the agreement between the electromyographical and the mechanomyographical methods and their relationship to clinical parameters of residual neuromuscular block. ⋯ However, due to the range of deviations between the EMG and the mechanomyogram, EMG TOF ratios down to 0.55 or up to 0.90 in some patients were found to be compatible with adequate recovery of a mechanomyographical TOF ratio of 0.75. Differences between electromyographical and mechanomyographical TOF ratios in their relation to 5 s sustained headlift were only revealed when the TOF ratio was below 0.50-0.60. Eight articles including 555 low back pain patients have been published. They included the following topics: 1) A ratio interval rating scale (Low Back Pain Rating Scale (RS)) was introduced. The possibility of registering the actual status in low back pain patients including; Back Pain, Sciatica, Functional Disability and Physical Impairment was studied. ⋯ The exercise programmes are generally free of side-effects. As regards chronic back pain patients with or without previous lumbar surgery, high dosage exercises with training periods lasting at least 12 to 16 sessions are of crucial importance for success. Exercises should be dynamic and full-range, and carried out following the adage "Don't let the pain be your guide.

The brain dopaminergic system. Pharmacological, behavioural and electrophysiological studies.
The kindling phenomenon is a good example of the effect of multiplicity on response increment processes in the nervous system. The electrical potentiation resembles pharmacological sensitization. An intermittent regimen is essential for a progressive augmentation of the behavioural response in both conditions. ⋯ Intermittent electrical activation of the cells in the VTA resulted in a syndrome characterized by a hypersensitive response to electrical or pharmacological dopaminergic provocation combined with abnormal social interactions. This new animal model may have implications for the understanding of the pathogenesis of schizophrenia. Hypotheses are proposed for the meaning of dopaminergic sensitization both in the development of persisting dyskinetic side-effects related to neuroleptic treatment and in the development of schizophrenia.
