Purpose: To evaluate the potential risk factors for poor dry eye disease (DED) outcomes after intense pulse light (IPL) treatment. Methods: A retrospective case-control study was conducted, and patients who received IPL were enrolled. A total of 63 eyes were included in the present study after exclusion and were divided into a fair outcome group and a poor outcome group according to posttreatment improvements in DED-related signs and symptoms. ⋯ Female sex and previous refractive surgery were associated with less improvement in DED-related symptoms (all P < 0.05), whereas advanced age, a lower pretreatment NITBUT and a higher meibomian gland loss rate were related to poor DED sign improvement (all P < 0.05). Conclusion: The major limitations are the retrospective design, small study population, and absence of detailed posttreatment exams. In conclusion, old age, female sex, previous refractive surgery, a lower NITBUT, and a higher meibomian gland loss rate are associated with worse outcomes after IPL treatment.

Background: This study aimed to develop a combined ultrasound (US)-pathology model to predict the axillary status more accurately after NST in breast cancer. Methods: This retrospective study included breast cancer patients who received NST at the First Affiliated Hospital of Nanjing Medical University from 2015 to 2022. Clinical, US, and pathological data were collected. ⋯ The US-based and combined US-pathology models were developed based on preoperative and postoperative information. The two models had an area under the receiver operating characteristic curve (AUC) of 0.821 and 0.883, respectively, which was significantly higher than that of the fine-needle aspiration model (AUC: 0.735). Conclusion: In this study, based on the US-based model, a combined model incorporating the characteristics of ALNs under US and breast pathology was developed and validated to predict axillary pCR.

Currently, there is no consensus on the treatment protocol for ossicular chain trauma. This study aims to investigate the classification and treatment strategies for traumatic ossicular chain dislocation. We retrospectively analyzed 15 patients. ⋯ Pneumolabyrinth was detected in 2 patients. We propose a novel classification system for traumatic ossicular chain dislocation. Treatment strategies should be tailored according to the specific trauma type.

Background: Sodium‒glucose cotransporter-2 (SGLT2) inhibitors offer glycaemic and cardiorenal benefits in the early stage of chronic kidney disease (CKD). However, the use of SGLT2 inhibitors may increase the risk of genitourinary tract infection (GUTI). Angiotensin-converting enzyme inhibitors (ACEIs) and angiotensin receptor blockers (ARBs) may also cause deterioration of kidney function. ⋯ Notably, SGLT2 inhibitor usage was associated with a greater GUTI incidence (aHR = 1.78, 95% CI = 1.122.84) shortly after initiation, irrespective of prior GUTI history status. Conclusion: Among patients with CKD stages 3-5, SGLT2 inhibitor use was linked to increased GUTI incidence, but it also significantly reduced the ESRD/dialysis risk without an episodic AKI or AKD risk. Clinical physicians should consider a personalized medicine approach by balancing GUTI episodes and cardiorenal outcomes for advanced CKD patients receiving SGLT2 inhibitors.

This study aimed to evaluate the clinical outcomes of patients diagnosed with acute severe hepatitis B (ASHB) who received early antiviral therapy compared to those who did not. Patients diagnosed with acute hepatitis B between February 2019 and February 2023 at our hospital were retrospectively analyzed for admission characteristics, antiviral treatments, and serum HBsAg and anti-HBs levels over 3-6-12 months. Acute severe hepatitis B was defined as serum total bilirubin > 5 mg or INR > 1.5. ⋯ Early antiviral therapy did not show an association with chronicity in ASHB patients. Conducting randomized controlled studies with a larger patient population is necessary to provide a definitive conclusion on initiating early antiviral therapy. However, such studies pose ethical challenges.
