Post-operative pain relief in children following caudal bupivacaine and buprenorphine--a comparative study.
Eighty-five paediatric patients (age range: 6 mths-12yrs) undergoing lower abdominal surgery were studied for post-operative pain relief following either caudal bupivacaine (GpI: n = 43) or buprenorphine (GpII: n = 42). Bupivacaine was administered as 0.5ml/kg body weight of 0.25% solution and buprenorphine as 4 micrograms/ml and volume of 0.5 ml/Kg body weight in normal saline. Post-operatively pain was graded on a 4-point scale and behaviour on a 5-point scale. ⋯ Bupivacaine provided good pain relief in the early post-operative hours but buprenorphine provided pain relief lasting for 24 hrs or more post-operatively. Post-operative behaviour of 10 patients receiving buprenorphine was graded as cheerful as compared to 2 from bupivacaine group. Till the end of observation period (i.e. 8 hr post-operatively), majority of patients receiving buprenorphine remained cheerful.

We present a study of fibrinogen levels in 133 patients who were prone to develop disseminated intravascular coagulation as a result of an underlying complication of pregnancy such as abruptio placentae, pregnancy-induced hypertension, missed abortion, septic abortion, intrauterine fetal death, vesicular mole and amniotic fluid embolism. A high incidence of hypofibrinogenemia was found in cases of abruptio placentae (43.9%) and pregnancy-induced hypertension (25%). ⋯ The use of this simple investigation makes possible the diagnosis of hemostatic failure and also helps to guide replacement therapy during the fibrinopenic state. There were 4 maternal deaths and 12 perinatal losses in this study.

The aim of this study was to assess whether 2 dimensional echocardiography can provide reliable parameters to quantitatively estimate pulmonary hypertension (PH) in patients with chronic obstructive pulmonary disease. Twenty subjects, 16 males and 4 females, mean age 54.5 year +/- 4.92 were evaluated. ⋯ A 2D Index was calculated RV x TA + Aw and the same correlated with mean pulmonary artery pressure (r = 0.97). This appears to be a useful non-invasive method foe estimating PH in cases of COPD.

Effect of induction-delivery and uterine-delivery on apgar scoring of the newborn.
Very short or prolonged induction-delivery interval (i.e. less than 5 minutes or more than 15 minutes) and uterine-delivery interval of more than 90 seconds has a definite effect on the apgar scoring of a newborn especially when general anaesthesia is administered as compared to regional anaesthesia for caesarean section.

Comparative Study
The value of ultrasonography in the diagnosis of adnexal masses.
Seventy patients with palpable adnexal masses were subjected to ultrasonographic examination. Three patients with negative ultrasound and no disease were excluded from the final analysis. ⋯ Ultrasonography is valuable in diagnosing functional and benign ovarian neoplasms. It is also useful in suspecting malignant ovarian neoplasms and confirming diagnosis of ectopic pregnancy, if correlated with the clinical findings.
