The purpose of this article is to examine the Institute of Medicine's recommendations, the National Prevention Council Action Plan, the medical home model, and the nursing standards that drive quality for telephone nursing triage. ⋯ Given the changing climate of the healthcare system, telephone nurses are capable of improving healthcare delivery in the twenty-first century.

The worldwide rate of medical errors and the subsequent patient harm triggered the healthcare organizations to find solutions to transform their punitive culture to safety culture. ⋯ The findings of the study suggested that patient safety educational interventions could be used by nursing administrators to enhance subcultures of patient safety such as "reporting" and "blame free," and to decrease the rate of adverse events.

The purpose of this paper is to discuss the impact of low health literacy and discuss interventions to minimize its effect on the elderly population. ⋯ Clinicians need to recognize the indicators of low health literacy and utilize the available assessment tools. To improve clinician-patient communication, interventions must be tailored to the patients' literacy level.
