Kounis syndrome also known as allergic myocardial infarction, represents the simultaneous occurrence of acute coronary syndromes with allergic or hypersensitivity reactions. We present a case of a 58-years-old male who developed anaphylaxis following a leech bite, leading to myocardial infarction despite the absence of prior allergic history. He was entubated and cardiopulmonary resusciation had been performed for 10 minutes. ⋯ The patient was stabilized and transferred to the intensive care unit and coronary angiography was performed, which revealed a 90% stenosis in the circumflex coronary artery. A stent was successfully implanted in the affected artery. This report emphasizes the complexity of diagnosing and managing Type-II Kounis syndrome and highlights the need for increased clinical awareness.

Infective endocarditis used to frequently cause mortality in subjects having PDA before the advent of antibiotics and surgical ligation. It has been documented that clinically silent PDAs may cause infective complications of heart valves. We present case of an 18-years-old male who presented with palpitations and fever to our emergency department. ⋯ A left thoracotomy approach was chosen for the PDA ligation that unveiled rupture of the main pulmonary artery, so the pulmonary artery repair alongside the PDA ligation was planned with median sternotomy approach and cardiopulmonary bypass immediately. The patient had a 14-days course of antibiotics during the ICU stay. In conclusion, infective endocarditis remains a rare yet life-threatening complication of PDA irrespective of the size; a timely PDA-ligation could prevent the life-threatening sequels.

To determine the effectiveness of Levamisole (Leva) in maintaining short-term and long-term remission in early steroid responders (ESRs) and late steroid responders (LSRs). ⋯ Levamisole was effective in maintaining short-term and long-term remission in early and late initial steroid responsive nephrotic syndrome.

This study examined the potential link between maternal pre-pregnancy body mass index (PPBMI) with neonatal outcomes in twin pregnancies. ⋯ Low PPBMI increases the risk of delivering LBW twins, independent of other factors. Our findings emphasize the importance of pre-pregnancy nutritional status in optimizing neonatal outcomes in twin deliveries.

In Pakistan, the real extent of fungal infection is unknown. Our objective was to estimate the burden of major fungal diseases here to emphasize their public health importance. ⋯ These findings show that fungal infections are a public health problem in Pakistan and that additional research is needed to assess their frequency in the general population. Clinicians must be taught and made aware of these infections to enhance diagnosis and treatment.
