The concept of editorial freedom or independence is examined in the light of the editor-owner relationship. Like individual and national freedom or independence, it is a rhetorical concept whose realisation flows from internal achievement as much as it depends on external validation. This freedom entails roles and responsibilities embodied in specific codes of practice for editors, such as the guidelines espoused by the International Committee of Medical Journal Editors and the World Association of Medical Editors. ⋯ The absence of clear contracts defining editorial independence and the lack of established mechanisms governing the editor-owner relationship affect many small- to medium-sized journals in developing countries. Even large journals in developed and democratic nations or totalitarian states and societies are not spared. At the end of the day, editorial freedom exists only insofar as it is tolerated, or until editors cross the line.

The incidence of mucocoeles associated with a non-surgically treated nasal polyposis is rare. We report a rare case of nasal polyposis with asymptomatic frontal mucocoeles in a 28-year-old Malay man who presented with bilateral nasal obstruction with anosmia. ⋯ Marsupialisation of the mucocoele and nasal polypectomy were done. Endoscopic sinus surgery and marsupialisation should be the treatment of choice for asymptomatic frontal mucocoele.

The aim of this study was to determine whether there was a difference in the pain indicators and effectiveness between venipuncture (VP) and heel prick (HP) for blood glucose monitoring in term neonates (recently, venipuncture was shown superior for the Guthrie test). ⋯ The HP is still the preferred method of drawing blood in neonates for blood glucose monitoring, as only one drop of blood is required.

The manuscript reviewer is a key player in the manuscript processing system and journal publication process. The peer review system is an important component of modern scientific publishing, and is a great help to editors in deciding whether or not a submitted manuscript is suitable for publication. The reviewer's responsibilities include protecting the integrity of his or her speciality or subspecialty, the reputation of the scientific journal, and the welfare of human and animal subjects, as well as treating the author's manuscript with respect, fairness and impartiality. The manuscript review should ideally be done in a systemic manner, with components comprising a brief summary and a tabulation of the manuscript analysis under the major headings of strengths, weaknesses, general comments and specific comments.
