[Quality criteria of assessment scales--Cohen's kappa as measure of interrator reliability (1)].
A far spread index for the quantification of the interrater-reliabilty of assessment instruments or procedures is the chance-corrected agreement measure "Cohen's Kappa". Although or even though Cohen's Kappa proves to be a common measure of agreement of two raters, it is increasingly being used uncritically. Many authors, but also readers don't seem to take the central and paradox characteristics of Kappa into consideration for the interpretation of their results. ⋯ This is in particular, when results of an investigation reveal particularly high values of observer agreement but low value of Cohen's Kappa or when differences in the overall assessment can even lead to an increase of Kappa. Only the knowledge about these characteristics makes a meaningful presentation and interpretation of the results of reliability studies possible. In a following second part of this paper, practicable alternatives to the problem areas presented and paradox characteristics of Kappa will be shown.

[Effects of kinesthetic mobilization in comparison with standard mobilization on respiratory function after coronary artery bypass and other relevant factors].
Presently no relevant nursing data exist in Germany on the effect of mobilization according to kinaesthetic principles on the physiological system, on locomotion and the sensory system. The effect of two different versions of mobilization was measured in a unicentric, open, randomized study with two parallel groups from December 1999 to October 2000 in the intermediate ICU for patients after heart surgery at Ulm University Hospital. The aim was, to show superiority of kinaesthetic mobilization, compared to empirically developed standard mobilization on patients after aortal coronary bypass surgery, with reference to respiratory function. ⋯ There was no diagnosis of pneumonia and none of instabile sternum on any of the subjects in this study. Also, there was no negative effect on the sternum due to the light support arms gave to sitting up and rotating movements in the area of thorax and pelvis on subjects in the kinaesthetic group. The subjects early trust in their own varied movement and the possible effects of kinaesthetic mobilization in later phases of recuperation was not explored in this study.

The research question was whether there exists a relationship between quality of nursing diagnoses in nursing documentations and patient's satisfaction with nursing diagnoses. The hypotheses stated that a positive relationship between nursing diagnosis and patient satisfaction exists; high quality of nursing diagnoses in the nursing documentation goes along with high patient satisfaction with nursing diagnoses. The sample consisted of 57 patients, their nursing documentations and nurses of three dermatological units of a university hospital in Switzerland. ⋯ Patients satisfaction with nursing diagnoses was rather high. One of three subscales in both instruments yielded a significant correlation. The statistically significant, but low relationship suggests the need for further study.
