Factors influencing the use of dietary interventions for modification of CKD risk among African Americans have not been well-explored. We assessed perceived barriers and facilitators of CKD prevention through dietary modifications among African Americans with low socioeconomic status (SES) and at high risk for CKD. ⋯ Low SES African Americans at high risk for CKD had limited perception of their risk but they identified multiple barriers and potential facilitators of CKD prevention via dietary modifications which can inform future studies and public health interventions.

The RaDIANT community study protocol: community-based participatory research for reducing disparities in access to kidney transplantation.
The Southeastern United States has the lowest kidney transplant rates in the nation, and racial disparities in kidney transplant access are concentrated in this region. The Southeastern Kidney Transplant Coalition (SEKTC) of Georgia, North Carolina, and South Carolina is an academic and community partnership that was formed with the mission to improve access to kidney transplantation and reduce disparities among African American (AA) end stage renal disease (ESRD) patients in the Southeastern United States. ⋯ The RaDIANT Community Study aims to improve equity in access to kidney transplantation for ESRD patients in the Southeast.

Proteasome inhibitors are a relatively new class of chemotherapeutic agents. Bortezomib is the first agent of this class and is currently being used for the treatment of multiple myeloma. However, recent reports have linked exposure to bortezomib with the development of thrombotic microangiopathy. A new agent in this class, carfilzomib, has been recently introduced as alternative therapy for relapsing and refractory multiple myeloma. We report a case of renal thrombotic microangiopathy associated with the use of carfilzomib in a patient with refractory multiple myeloma. ⋯ In view of the previously reported association of bortezomib with thrombotic microangiopathy, the temporal association of the clinical picture with the initiation of carfilzomib, and the partial resolution of symptoms after discontinuation of the drug, we conclude that carfilzomib may have precipitated a case of clinically evident renal thrombotic microangiopathy in our patient.

Since October 2010, the combination of a heparin-grafted polyacrilonitrile (AN69ST) membrane with a 0.80 mmol/L citric acid-containing dialysate is routinely used in our centre for intermittent haemodialysis, without systemic anticoagulation, in critically ill patients with increased bleeding risk. The primary outcome of this retrospective cohort study was to assess the development of circuit clotting during these dialysis procedures. Secondly, we assessed the impact of clotting on treatment duration, the incidence rate of coagulation-induced retransfusion failure and the association of patient and dialysis characteristics with the occurrence of clotting. ⋯ Intermittent haemodialysis without systemic anticoagulation combining a heparin-grafted AN69ST dialyzer with a citrate-enriched dialysate favourably compares as to clotting complications with the published outcomes of anticoagulation-free intermittent haemodialysis strategies using saline flushes, heparin-coated dialyzer in combination with regular dialysate or regional citrate anticoagulation with calcium supplemented dialysate. The incidence of circuit clotting in our cohort appears to be higher than previously reported for regional citrate anticoagulation with a calcium-free dialysate.

Fabry disease is a rare, X-linked, lysosomal storage disease caused by mutations in the gene encoding the enzyme alpha-galactosidase A. Complete or partial deficiency in this enzyme leads to intracellular accumulation of globotriaosylceramide (Gb3) and related glycosphingolipids in many cell types throughout the body, including the kidney. Progressive accumulation of Gb3 in podocytes, epithelial cells and the tubular cells of the distal tubule and loop of Henle contribute to the renal symptoms of Fabry disease, which manifest as proteinuria and reduced glomerular filtration rate leading to chronic kidney disease and progression to end-stage renal disease. ⋯ Several strategies are required to complement the use of ERT and treat the myriad of associated symptoms and organ involvements. In particular, patients with renal Fabry disease are at risk of cardiovascular events, such as high blood pressure, cardiac arrhythmias and stroke. This review discusses the management of renal involvement in Fabry disease, including diagnosis, treatments, and follow-up, and explores recent advances in the use of biomarkers to assist with diagnosis, monitoring disease progression and response to treatment.
